铒点阵激光联合A型肉毒毒素注射治疗眼周皱纹的临床疗效及安全性

Clinical Efficacy and Safety of Erbium Fractional Laser Combined with Injection of Botulinum Toxin Type A in Treatment of Periocular Wrinkles

目的探讨铒点阵激光联合A型肉毒毒素注射治疗眼周皱纹的临床疗效及安全性。方法回顾性分析2020年2月至2022年2月首都医科大学大兴教学医院皮肤科治疗的80例有明显眼周皱纹患者的临床资料,根据治疗方法不同分为观察组和对照组,每组40例。观察组先接受铒点阵激光对眼周皮肤进行治疗,再接受A型肉毒毒素对皱纹注射治疗;对照组仅接受A型肉毒毒素对皱纹注射治疗。治疗后7 d、1个月、3个月、6个月,对两组患者眼周皱纹改善情况进行面部整体美容程度改善量表(GAIS)、优良率及满意度评价。结果两组患者术后均未出现恶化,观察组术后7 d、1个月、3个月、6个月静态皱纹GAIS评价效果优于对照组(P<0.01);两组患者术后不同时间动态皱纹的GAIS评价效果比较差异无统计学意义(P>0.05)。观察组术后7 d、1个月、3个月、6个月静态皱纹改善优良率均明显高于对照组(P<0.05或P<0.01),两组患者术后不同时间动态皱纹改善优良率比较差异无统计学意义(P>0.05)。治疗后3个月和6个月,观察患者满意度优于对照组(P<0.01)。两组患者均未出现严重的不良反应,主要不适感为轻微的疼痛,临床表现则为治疗区域轻度泛红、偶有淤青以及轻度肿胀,治疗后1周内基本消失。结论铒点阵激光联合A型肉毒毒素注射治疗改善眼周皱纹的整体效果明显,患者满意度高,仅有轻微的不良反应。

Objective To investigate the clinical efficacy and safety of erbium fractional laser combined with injection of botulinum toxin type A in the treatment of periocular wrinkles.Methods The clinical data of 80 patients with obvious periocular wrinkles treated by Department of Dermatology of Daxing Teaching Hospital of Capital Medical University from Feb.2020 to Feb.2022 were retrospectively analyzed.They were divided into an observation group and a control group according to different treatment methods,with 40 cases each.The observation group received erbium fractional laser combined with injection treatment of botulinum toxin type A;the control group only received botulinum toxin type A treatment.The patients in each group were followed up(at 7 days and 1,3,6 months)after treatment.The treatment results of periocular wrinkles in the two groups were evaluated by the global aesthetic improvement scale(GAIS),excellent and good rate,and satisfaction.Results No deterioration occurred in both groups after operation.The GAIS evaluation of static wrinkles in the observation group was better than that in the control group at 7 d,1 month,3 months and 6 months after surgery(P<0.01).There was no significant difference between the two groups in GAIS evaluation of dynamic wrinkles at different time points after surgery(P>0.05).The excellent and good improvement rates of static wrinkles in the observation group were significantly higher than that in the control group at 7 d,1 month,3 months and 6 months after surgery(P<0.05 or P<0.01),and there was no significant difference in the excellent and good improvement rate of dynamic wrinkles at different time points after surgery between the two groups(P>0.05).At 3 and 6 months after treatment,the satisfaction of the observation group was better than that of the control group(P<0.01).There were no serious adverse reactions in both groups.The main discomfort was mild pain,and the clinical manifestations were mild redness,occasional bruising and mild swelling in the treatment area,which basically disappeared within 1 week after treatment.Conclusion Erbium fractional laser combined with injection of botulinum toxin type A has obvious overall effect in improving periocular wrinkles,with high patient satisfaction and only mild adverse reactions.
