摘要
目的分析宫颈病变组织中磷脂酰肌醇3激酶(PI3K)-蛋白激酶B(Akt)信号通路相关蛋白的表达及与人乳头瘤病毒(HPV)感染的关系.方法选择2019年2月-2021年12月福建省妇幼保健院收治的子宫颈鳞状细胞癌患者87例纳入子宫颈鳞状细胞癌组,另选择同期慢性子宫颈炎患者96例纳入慢性子宫颈炎组、子宫颈上皮内瘤变患者92例纳入子宫颈上皮内瘤变组;比较三组患者PI3K、Akt、核因子κB(NF-κB)阳性表达情况,检测不同病理情况子宫颈鳞状细胞癌患者PI3K、Akt、NF-κB阳性表达及HPV感染情况.通过Spearman相关性分析法分析PI3K-Akt信号通路相关蛋白的表达(PI3K、Akt、NF-κB)与HPV感染的相关性.结果慢性子宫颈炎组、子宫颈上皮内瘤变组、子宫颈鳞状细胞癌组宫颈组织PI3K、Akt、NF-κB阳性表达情况呈升高趋势(P<0.05);国际妇产科联盟(FIGO)分期Ⅲ~Ⅳ级、组织学分级低分化的宫颈癌患者宫颈组织中PI3K、Akt、NF-κB阳性表达率更高,淋巴结转移NF-κB阳性表达率更高(P<0.05);子宫颈鳞状细胞癌患者87例,其中HPV阳性74例,阳性率85.06%;FIGO分期Ⅲ~Ⅳ级、组织学分级低分化患者HPV感染率更高(P<0.05);Spearman相关性分析法结果显示,HPV与PI3K、Akt、NF-κB表达呈正相关(r=0.681、0.639、0.877,P均<0.05),PI3K与Akt、NF-κB表达呈正相关(r=0.708、0.742,P均<0.05);Akt与NF-κB表达呈正相关(r=0.637,P=0.009).结论PI3K-Akt信号通路可参与慢性子宫颈炎-子宫颈上皮内瘤变-子宫颈鳞状细胞癌的病情发展,宫颈组织PI3K-Akt信号通路相关蛋白表达及与HPV感染密切相关,可能作为临床诊治的新靶点.
OBJECTIVE To analyze the expression of phosphatidylinositol 3 kinase(PI3K)-protein kinase B(Akt)signaling pathway-related proteins and their association with human papillomavirus(HPV)infection in cervical lesions.METHODS Totally 87 patients with cervical squamous cell carcinoma admitted to Fujian Maternity and Child Health Hospital from Feb.2019 to Dec.2021 were selected and included in the cervical squamous cell carcinoma group,96 patients with chronic cervicitis during the same period were included in the chronic cervicitis group and 92 patients with cervical intraepithelial neoplasia were included in the cervical intraepithelial neoplasia group.The positive expression of PI3K,Akt and nuclear factor κB(NF-κB)in the three groups of patients were compared,and the positive expression of PI3K,Akt and NF-κB and HPV infection in patients with cervical squamous cell carcinoma in different pathological conditions were detected.Spearman correlation analysis was used to analyze the correlation between the expression of PI3K-Akt signaling pathway-related proteins(PI3K,Akt,NF-κB)and HPV infection.RESULTS The positive expressions of PI3K,Akt and NF-κB in the cervical tissues of patients in the chronic cervicitis group,the cervical intraepithelial neoplasia group and the cervical squamous cell carcinoma group showed an increasing trend(P<0.05).The positive expression rate of PI3K,Akt and NF-B in the cervical tissues of patients with Federation of International of Gynecologists and Obstetricians(FIGO)stage II-IV poorly differentiated histological grading was significantly increased,and the positive expression rate of NF-κB in lymph node metastasis was increased(P<0.05).Out of the 87 patients with cervical squamous cell carcinoma,74 cases were detected as HPV positive,with a positive rate of 85.06%.The HPV infection rate was higher in cervical cancer patient with in FIGO stage II-IV and poorly differentiated histological grading(P<0.05).Spearman correlation analysis showed that HPV was positively correlated with the expression of PI3K,Akt and NF-κB(r=0.681,0.639,0.877,all P<0.05),and PI3K was positively correlated with the expression of Akt and NF-κB(r=0.708,0.742,P all<0.05).Akt was positively correlated with NF-κB expression(r=0.637,P=0.009).CONCLUSION PI3K-Akt signaling pathway was involved in the development of chronic cervicitis,cervical intraepithelial neoplasia and cervical squamous cell carcinoma,the expression of PI3K-Akt signaling pathway-related proteins in cervical tissues was closely related to HPV infection,which might be new target for clinical diagnosis and treatment.
