腹泻型肠易激综合征发病机制及中医药治疗的研究进展

Research Progress in Pathogenesis and Traditional Chinese Medicine Treatment of Diarrheal Irritable Bowel Syndrome

腹泻型肠易激综合征(IBS-D)是一种慢性胃肠道功能紊乱性疾病,也是消化内科门诊量逐年上升的原因。IBS-D的发生发展与遗传、感染、精神、饮食、神经和内分泌等因素密切相关。IBS-D的发病机制复杂,主要涉及炎症、中枢神经系统、消化系统、免疫系统等。中医药具有多成分、多通路及多靶点的特点,不仅临床疗效好、复发率低,且不良反应少。中医药不仅可以调控肠道菌群,还可以调节微RNA(miRNA),从而治疗IBS-D。未来从miRNA与中医药联合探索IBS-D的发病机制,可为IBS-D的治疗提供新思路。

Diarrheal irritable bowel syndrome(IBS-D)is a chronic gastrointestinal dysfunction disease,which is also the reason why the number of gastroenterology consultations is increasing year by year.The occurrence and development of IBS-D are closely related to genetic,infectious,psychological,dietary,neurological and endocrine factors.The pathogenesis of IBS-D is complex,mainly involving inflammation,central nervous system,digestive system,immune system,etc.Traditional Chinese medicine(TCM)has the characteristics of multiple components,pathways and targets,which not only has good clinical efficacy,low recurrence rate,but also has fewer adverse reactions.TCM can regulate not only the gut microbiota,but also microRNA(miRNA)to treat IBS-D.In the future,exploring the pathogenesis of IBS-D through the combination of miRNA and TCM can provide new insights for the treatment of IBS-D.
