胃黏膜保护剂在慢性胃炎治疗中的应用进展

Application Progress of Gastric Mucosal Protective Agents in Treatment of Chronic Gastritis

慢性胃炎是由多种致病因素引起的消化系统疾病,主要表现为胃黏膜炎症性改变,部分患者还会发生胃黏膜固有层腺体萎缩、上皮内瘤变,严重者可进展为胃癌,威胁患者生命健康。慢性胃炎的临床治疗以药物为主,常见的药物包括质子泵抑制剂、促胃动力药、抗酸剂和胃黏膜保护剂等。其中胃黏膜保护剂不仅具有抑酸、抑制胃蛋白酶活力的作用,还能促进细胞更新、增加胃黏膜层血流供应,从而发挥黏膜保护、促进黏膜修复的作用,逐渐应用于慢性胃炎的治疗。目前,用于慢性胃炎治疗的胃黏膜保护剂有铋剂类、硫氢键类等,未来在临床研究基础上,更多胃黏膜保护剂治疗慢性胃炎的循证医学证据将为慢性胃炎的治疗提供新方向。

Chronic gastritis is a digestive system disease caused by a variety of pathogenic factors,mainly manifested as inflammatory changes of gastric mucosa.Some patients also have atrophy of glands in the lamina propria of gastric mucosa and intraepithelial neoplasia,which can progress to gastric cancer in severe cases,threatening the life and health of the patients.The clinical treatment of chronic gastritis is mainly based on drugs.Common drugs include proton pump inhibitors,gastric motility drugs,antacids and gastric mucosal protective agents.Among them,gastric mucosal protective agents not only have the functions of inhibiting acid and pepsin activity,but also can promote cell renewal and increase the blood flow supply of gastric mucosal layer,so as to play the role of mucosal protection and promoting mucosal repair,which are gradually being applied to the treatment of chronic gastritis.At present,gastric mucosal protective agents used for the treatment of chronic gastritis include bismuth agents,sulfur hydrogen bonds,etc.In the future,on the basis of clinical research,more evidence of evidence-based medicine for the treatment of chronic gastritis with gastric mucosal protective agents will provide new therapeutic directions.
