摘要
肝移植是目前治疗终末期肝病的有效手段。目前边缘供肝已大量用于解决供体短缺问题,但其易受缺血再灌注损伤,从而增加术后并发症的发生风险。机械灌注可模拟肝脏在机体内的生理条件,保存供肝、改善供肝质量、评估供肝功能。机械灌注虽可显著改善供肝保存技术,在静态冷保存方面优势显著,但目前尚处于临床试验阶段,主要技术参数也尚未统一。因此,未来优化灌注参数,完善供肝功能评估指标,使机械灌注大规模应用于临床,可以为边缘供肝移植提供新思路。
Liver transplantation is an effective treatment for end-stage liver diseases.Marginal donor livers have been widely used to solve the problem of donor shortage,which may increase the risk of postoperative complications because they are prone to ischemia-reperfusion injury.Mechanical perfusion,simulating the physiological conditions of the liver in the body,can preserve the donor livers,improve the quality of the donor livers and assess the function of the donor livers.Although mechanical perfusion can significantly improve the techniques of donor livers preservation and has more advantages in static cold preservation,it is still in clinical trial stage and the main technical parameters have not been unified.Hence,with the optimization of perfusion parametersand the improvement of evaluation indicators of donor liver function,mechanical perfusion can be used clinically on a large scale,bringing new ideas for marginal donor livers transplantation.
