宫颈癌带尿管出院病人的延续性集束化护理策略对降低尿路感染的影响研究

Impact of continuous bundled nursing strategy on reducing urinary tract infection for cervical cancer patients discharged with urinary catheter

目的:探讨延续性集束化护理策略对降低带尿管出院的宫颈癌病人尿路感染的影响。方法:抽取2019年1月—12月收治的行宫颈癌根治术的病人46例为对照组;抽取2020年1月—2022年6月收治的行宫颈癌根治术的病人111例为干预组,行回顾性队列分析。对照组采用常规护理措施,干预组在常规护理措施基础上,院内集束化护理,院外继续给予云随访等延续性干预措施,并统计术后3、5、7、10、14、20 d病人导尿管总感染率、平均留置导尿时间、其他导尿管相关并发症发生率、住院时长作为干预评价指标。结果:干预组宫颈癌病人平均住院日缩短;术后尿路总感染率、平均留置导尿时间、其他导尿管相关并发症发生率均下降,经比较差异有统计学意义(P<0.05)。结论:延续性集束化护理策略可以有效降低宫颈癌带尿管出院病人的尿路总感染率、减少平均留置尿管时间、降低导尿管相关并发症发生率、减少住院时间。

Objective:To explore the effect of a continuous bundled nursing strategy on reducing urinary tract infections in cervical cancer patients discharged with urinary catheters.Methods:A total of 46 patients who underwent radical cervical cancer surgery from January to December 2019 were selected as the control group;another 111 patients who underwent radical cervical cancer surgery from January 2020 to June 2022 were selected as the intervention group.Retrospective cohort analysis was conducted in this study.The patients in control group adopted routine nursing measures,and the patients in intervention group received continuous bundled intervention nursing measures based on routine nursing measures after discharge.The total catheter infection rate,average indwelling catheterization time,the incidence of other catheter-related complications,and patient′s length of stay(LOS)on the 3rd,5th,7th,10th,14th,and 20th days after surgery were analyzed as intervention evaluation indicators.Results:The average LOS of cervical cancer patients in the intervention group were shortened;the total postoperative urinary tract infection rate,the average indwelling catheterization time,and the incidence of other catheter-related complications were all reduced,and the differences were statistically significant(P<0.05).Conclusions:The continuous bundled nursing strategy could effectively reduce the total urinary tract infection rate,average indwelling time of urinary catheters,incidence of catheter-related complications,and LOS of cervical cancer patients discharged with urinary catheters.
