中医适宜技术在脑卒中后伴胃肠道功能障碍患者中的应用

Application of traditional Chinese medicine appropriate techniques in patients with post-stroke gastrointestinal dysfunction

目的探讨中医适宜技术在脑卒中康复期患者胃肠道功能障碍治疗中的应用效果。方法选取2020年7月至2022年7月杭州市中医院收治的84例脑卒中康复期患者为研究对象,采取计算机随机数字表法分为研究组(42例)和对照组(42例)。对照组接受常规护理进行胃肠道管理,研究组接受中医适宜技术进行胃肠道管理。比较2组在干预30 d后的胃肠道功能障碍恢复效果,日常生活活动能力(改良Barthel指数)、不良情绪(汉密尔顿焦虑量表)、生活质量[健康状况调查量表36(SF-36)]、Bristol粪便性状量表和Wexner失禁评分。结果干预后,2组的改良Barthel指数评分均高于干预前(P均<0.05),且研究组均高于对照组(P<0.05)。干预后,2组的汉密尔顿焦虑量表评分均低于干预前(P均<0.05),且研究组均低于对照组(P<0.05)。干预后,2组的SF-36评分均高于干预前(P均<0.05),且研究组高于对照组(P<0.05)。干预后,观察组的Bristol粪便性状量表和Wexner失禁评分均较对照组更优(P均<0.05)。结论在脑卒中康复期患者胃肠道功能障碍治疗中应用中医适宜技术,可提高其胃肠道功能恢复效果、生活质量和日常生活活动能力,缓解其焦虑情绪。

Objective To investigate the effect of the application of Traditional Chinese Medicine techniques in the management of gastrointestinal dysfunction among post-stroke rehabilitation patients.Methods Eighty-four post-stroke rehabilitation patients admitted to Hangzhou Hospital of Traditional Chinese Medicine from July 2020 to July 2022 were selected as the research subjects.They were randomly divided into an intervention group(n=42)and a control group(n=42)using a computer random number table method.The control group received routine care for gastrointestinal management,while the intervention group received traditional Chinese medicine appropriate techniques for gastrointestinal management.The recovery effect of gastrointestinal dysfunction daily living activities ability(Modified Barthel Index),negative emotions(measured by the Hamilton Anxiety Scale),and quality of life(measured by the Short Form Health Survey-36[SF-36],Bristol Stool Form Scale,and Wexner Incontinence Score)were assessed 30 days after intervention.Results After intervention,the scores of the Modified Barthel Index in the intervention group were higher than those in the control group and those before intervention(P<0.05).The scores of the Hamilton Anxiety Scale in the intervention group were lower than those in the control group and those before intervention(P<0.05).The SF-36 scores in the intervention group were higher than those in the control group and those before intervention(P<0.05).The scores of Bristol Stool Form Scale and Wexner Incontinence Score of the observation group were better than those of the control group after intervention(P<0.05).Conclusion The application of traditional Chinese medicine appropriate techniques in the management of gastrointestinal dysfunction in post-stroke rehabilitation patients can improve gastrointestinal function recovery,quality of life,and daily living activities ability,and alleviate anxiety.
