胃癌患者饮食蛋白质摄入量现状及其对营养状况影响的横断面研究

摘要 目的本研究旨在探讨胃癌(GC)患者营养不良的影响因素。方法对2019年1~5月在上海某三甲医院接受治疗的GC患者进行横断面研究,收集人口统计学、疾病相关和营养相关信息的数据,使用患者主观整体评估量表评估营养状况,并使用食物频率问卷评估饮食摄入量。将单因素分析结果中至少P<0.1的变量纳入Logistic回归模型,分析导致患者营养不良的相关因素。结果共纳入160例GC患者,其中70.6%的患者存在营养不良,21.9%的患者能量摄入不足,32.5%的患者蛋白质摄入不足。多因素Logistic回归分析结果显示,年龄≥65岁(OR=3.62,P=0.005)和蛋白质摄入量<2 g/(kg·d)(OR=3.01,P=0.005)是导致GC患者营养不良的主要因素。结论研究结果表明,GC患者仍然普遍存在营养不良,年龄和蛋白质摄入不足是导致患者营养不良的危险因素,医护人员在对胃癌合并营养不良的患者进行营养宣教时,应注意饮食中蛋白质的摄入。 Objective This study aimed to explore factors contributing to malnutrition in gastric cancer(GC)patients.Methods A cross-sectional study was conducted on GC patients admitted between January and May 2019 at a Grade A tertiary hospital in Shanghai,China.Data on demographic,disease-related and nutritional-related information were collected.Specifically,nutrition status was assessed using patient-generated subjective global assessment,and dietary intake was assessed by food frequency questionnaire.Variables with P value less than 0.1 on univariate analysis went on to be included in multiple logistic analysis to identify the risk factors to malnutrition.Results A total of 160 GC patients were analyzed in this study.Overall,70.6%of patients were malnourished.21.9%of GC patients did not reach energy intake target,while 32.5%of GC patients did not reach protein intake target.The results of logistic regression showed that age 65 or greater(OR=3.62,P=0.005)and protein intake less than 2 g/(kg·d)(OR=3.01,P=0.018)were contributing factors to GC patient malnutrition.Conclusion The study results suggested that malnutrition is still prevalent in GC patients.Age and under target protein intake were the potential risk factors to malnutrition.Clinician should pay attention to dietary protein intake when providing nutritional counselling for gastric cancer patients with malnutrition.
