摘要
[目的]研究我国2015-2019年中医类医院服务效率的空间分布特征、区域差异及动态变化规律,分析其服务效率的空间溢出效应和影响因素,为促进中医服务高质量发展提供依据。[方法]使用DEA-BCC模型和Malmquist指数测算我国中医类医院静态和动态服务效率,利用空间计量方法对中医类医院服务效率的空间分布、聚集特征及其影响因素进行分析。[结果]5年间,我国中医类医院全要素生产率下降且受规模效率制约;西南华中地区整体效率较高并且呈高-高聚集特点,华北地区整体效率偏低且呈低-低聚集;整体空间不均衡性且有所缩小;中医类医院服务效率在相邻省份间存在正向空间溢出效应;城镇化、老龄化、中医药财政投入、医护比、中医类医师占比是中医类医院服务效率主要影响因素(P<0.05)。[结论]我国中医类医院服务效率不高,存在空间相关性、呈现空间聚集性,建议平衡区域中医资源投入规模,加强区域性中医发展统筹规划,发挥区域协同作用。
Objective This paper studies characteristics of spatial distribution,regional differences and dynamic change rules of service efficiency of TCM hospitals in China from 2015 to 2019,analyzes the spatial spillover effect and influencing factors of the service efficiency,and provides a basis for promoting the high-quality development of TCM services.Methods The DEA-BCC model and Malmquist index were used to measure the static and dynamic service efficiency of TCM hospitals.The spatial distribution,clustering characteristics and influencing factors of the service efficiency of TCM hospitals were analyzed by the spatial econometrics.Results The total factor productivity of TCM hospitals decreased and was restricted by scale efficiency in the five years.The overall efficiencies in southwest and central China were high and had the characteristics of high-high aggregation;the overall efficiency in north China was low and had the characteristics of low-low aggregation.The overall spatial imbalance had narrowed.The service efficiency of TCM hospitals had the positive spatial spillover effect among neighboring provinces.Urbanization,aging,financial investment in traditional Chinese medicine,the ratio of doctors to nurses,and the proportion of traditional Chinese medicine doctors were the main factors affecting the service efficiency of TCM hospitals(P<0.05).Conclusions The service efficiency of TCM hospitals in China is not high,and there is spatial correlation and spatial clustering.It is suggested to balance the investment scale of regional TCM resources,strengthen the overall planning of regional TCM development,and leverage regional synergies.
