摘要
目的:降低医院肠内营养不合理医嘱的发生率,促进肠内营养特医食品(FSMP)的合理使用。方法:以我院2019年1-12月的肠内营养医嘱300例为未改善组,300例收治时间为2020年7月至9月的使用肠内营养制剂的患者为改善组。统计未改善组不合理医嘱数量及发生类型,对存在的问题进行分析,运用FOCUS-PDCA循环管理法对改善组患者的肠内营养特医食品医嘱进行管理,统计管理后的肠内营养医嘱,分析比较不合理医嘱数量及发生类型、住院患者死亡率及好转出院率,评价管理效果。结果:与未改善组相比,改善组FSMP不合理医嘱发生率降低,住院患者死亡率降低,好转出院率升高。结论:采用FOCUS-PDCA循环管理可降低肠内营养不合理医嘱发生率,促进医院肠内营养特医食品的合理使用。
Objective:To reduce the incidence of unreasonable medical advice on enteral nutrition in our hospital,and to promote the rational use of enteral nutrition special medical food.Method:A total of 300 cases of enteral nutrition orders in our hospital from January to December 2019 were collected as the unimproved group,and 300 cases of patients receiving enteral nutrition preparations from July to September 2020 were collected as the improved group.The number and types of unreasonable medical orders were counted and the existing problems were analyzed.Then,the FOCUS-PDCA cycle management method was used to manage the enteral nutrition special medical food orders of patients in the improvement group.After management,the enteral nutrition orders were collected,and the number and occurrence types of unreasonable medical orders,the mortality rate of inpatients and the rate of improved discharge were statistically analyzed to evaluate the management effect.Results:The incidence of unreasonable medical advice of FSMP was sig⁃nificantly decreased in the improved group compared with the non-improved group.The mortality rate of inpatients decreased,and the rate of improved discharge was higher than that before the implementation of management.Conclusion:The FOCUS-PDCA cycle man⁃agement can reduce the incidence of unreasonable medical orders for enteral nutrition and promote the rational use of special enteral nutrition food in hospitals.
