调强适形放疗联合化疗对老年晚期宫颈癌患者生活质量及免疫功能和疗效的影响

Effect of intensity-modulated radiotherapy combined with chemotherapy on quality of life,immune function and curative effect in elderly patients with advanced cervical cancer

目的 探讨调强适形放疗(intensity-modulated radiotherapy, IMRT)联合化疗治疗老年晚期宫颈癌的临床疗效及对免疫功能与生活质量的临床影响。方法 选取2018年1月—2019年1月郑州大学第一附属医院收治的老年晚期宫颈癌患者80例,依据患者所选治疗方式分组,对照组(n=42)采用IMRT,观察组(n=38)采用IMRT联合化疗,比较两组临床疗效、生活质量及免疫功能。结果 观察组ORR率为84.21%,随访3年复发率为10.53%,转移率为2.63%,与对照组(76.19%、19.05%、9.52%)比较差异无统计学意义(P>0.05);两组治疗前免疫功能与QOL评分比较差异无统计学意义(P>0.05),治疗后观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)明显低于同组治疗前且观察组明显高于对照组(P<0.001),QOL评分明显高于同组治疗前且观察组明显高于对照组(P<0.001)。结论 调强适形放疗联合化疗治疗老年晚期宫颈癌患者临床疗效较好,可更好保护患者的免疫功能并改善其生活质量,有推广价值。

Objective To investigate the clinical efficacy of intensity-modulated radiotherapy combined with chemother-apy in the treatment of advanced cervical cancer in the elderly and its clinical effect on immune function and quality of life.Methods Eighty elderly patients with advanced cervical cancer admitted to the Department of Gynecology of our hospital from January 2018 to January 2019 were divided into two groups according to the selected treatment methods.The control group(n=42)received IMRT,and the observation group(n=38)received IMRT combined with chemo-therapy.The clinical efficacy,quality of life and immune function of the two groups were compared.Results The ORR rate was 84.21%,the recurrence rate was 10.53%and the metastasis rate was 2.63%in the observation group,which were not significantly different from 76.19%,19.05%and 9.52%in the control group(P>0.05).There was no sig-nificant difference in immune function and QOL score between the two groups before treatment(P>0.05).After treat-ment,CD3^(+),CD4^(+),CD4^(+)/CD8^(+) in the observation group were significantly lower than those before treatment,while the observation group was significantly higher than the control group(P<0.001).The QOL score of the observation group was significantly higher than that of the control group(P<0.001).Conclusions Intensity-modulated radio-therapy combined with chemotherapy has good clinical efficacy in the treatment of elderly patients with advanced cervical cancer,which can effectively enhance the immune function of patients and improve their quality of life,and has the value of promotion.
