中圆针在胃癌根治术消化道重建中的应用

Application of middle round needle in digestive tract reconstructionin radical gastrectomy for gastric cancer

目的 评估中圆针缝合在胃癌根治术中胃空肠吻合口加针的有效性和安全性。方法 144例胃癌患者,其中男性68例,女性76例,远端胃患者102例,近端胃患者42例,均接受胃癌根治术,术中采用中圆针加固胃空肠吻合口(A组)73例,采用小圆针加固胃空肠吻合口(B组)71例。比较两组患者术中及术后的情况。结果 两组患者均顺利完成手术,A、B组术中加固吻合口中出血量、术中注水实验针孔渗漏率、术后吻合口瘘发生率、吻合口出血发生率比较,差异均无统计学意义(P>0.05)。A组术中加固胃空肠吻合口需要时间为(1.8±0.7)min、B组所需时间为(2.3±0.7)min。A组低于B组(P<0.05)。结论 中圆针缝合是一种安全、可行且更为快捷的胃空肠吻合口加针的方法。

Objective To evaluate the efficacy and safety of middle round needle suture in gastrojejunal anastomosis in radical gastrectomy.Methods A total of 144 patients with gastric cancer,including 68 males and 76 females,102 pa-tients with distal gastric cancer and 42 patients with proximal gastric cancer,underwent radical gastrectomy.During op-eration,73 patients were treated with middle round needle(group A)and 71 patients were treated with small round nee-dle(group B).The intraoperative and postoperative conditions of the two groups were compared.Results All the pa-tients in the two groups successfully completed the operation.There were no significant differences in intraoperative rein-forcement anastomotic bleeding volume,intraoperative water injection test pinhole leakage rate,postoperative anastomot-ic leakage rate,anastomotic bleeding rate between group A and group B(P>0.05).The time required for reinforce-ment of gastrojejunal anastomosis in group A was(1.8±0.7)minutes,and that in group B was(2.3±0.7)minutes.Group A was lower than group B(P<0.05).Conclusion The middle round needle suture is a safe,feasible and faster method for gastrojejunostomy.
