甲状腺癌根治术联合甲状旁腺自体移植的临床观察

Clinical observation of radical thyroidectomy combined withparathyroid autotransplantation

目的 观察分析甲状腺癌根治术联合甲状旁腺自体移植的临床应用价值。方法 选取2021年1月—2023年1月驻马店市中心医院甲状腺外科收治的90例甲状腺癌患者为研究对象,随机分为对照组与研究组两组(n=45),分别实施甲状腺根治术、甲状腺癌根治术联合甲状旁腺自体移植,分析两组患者的临床治疗情况。结果 研究组患者的临床治疗效果(总有效率95.56%)明显优于对照组(总有效率82.22%),研究组在感觉异常、腕-足痉挛、肢/指端麻木、焦虑方面发生率低于对照组,研究组术后1个月、3个月、半年甲状旁腺激素(PTH)水平均高于对照组,研究组的短暂性甲状旁腺功能减退(HP)、永久性HP发生率均低于对照组,上述各指标的组间比较差异均有统计学意义(P<0.05)。结论 将甲状腺癌根治术联合甲状旁腺自体移植用于甲状腺癌的治疗中,可以改善甲状腺功能,促进甲状旁腺激素的分泌,降低HP的发生风险,显示出较好的治疗效果。

Objective To observe and analyze the clinical application value of radical thyroidectomy combined with parathyroid autotransplantation.Methods Ninety patients with thyroid cancer admitted to the Department of Thy-roid Surgery of our hospital from January 2021 to January 2023 were selected as the study objects,and randomly di-vided into two groups(n=45),respectively,radical thyroidectomy,radical thyroidectomy combined with parathy-roid autotransplantation,and analyzed the clinical treatment of the two groups of patients.Results The clinical treatment effect(total effective rate 95.56%)of the study group was significantly better than that of the control group(total effective rate 82.22%).The incidence of paresthesia,carpo-foot spasm,limb/finger numbness and anxiety in the study group was lower than that in the control group.The level of parathyroid hormone(PTH)in the study group was higher than that in the control group at 1 month,3 months and 6 months after surgery.The inci-dence rates of transient hypoparathyroidism(HP)and permanent HP in the study group were lower than those in the control group,and the differences among the above indexes were statistically significant(P<0.05).Conclusion Radical thyroidectomy combined with parathyroid autotransplantation in the treatment of thyroid canc-er can improve thyroid function,promote the secretion of parathyroid hormone,reduce the risk of HP,and show a good therapeutic effect.
