依奇珠单抗治疗银屑病合并尿毒症1例

Ixekizumab in Treatment of Plaque Psoriasis with Uremia:A Case Report

患者男,64岁,全身红斑、脱屑伴瘙痒11年。皮损组织病理示:角化不全,棘层增厚,表皮突整齐向下延伸,真皮乳头毛细血管扩张,周围可见淋巴细胞浸润。银屑病面积和严重程度指数(PASI)评分14.4分;体表受累面积评分(BSA)32%;银屑病静态临床医生整体评估(PGA)4分。诊断为斑块型银屑病。患者7年前因尿毒症行血液透析,期间经多种药物治疗,皮疹无明显好转。治疗:给予依奇珠单抗皮下注射治疗,患者皮疹显著改善,随访中未见不良反应。

A 64-year-old male presented with erythematous-desquamative plaques all over the body for eleven years.Histopathology of the lesions showed parakeratosis,thickening of the spinous layer,extended downward growth of the epidermal protrusions,dilated capillaries in the dermal papillae,and lymphocytic infiltration surrounding the lesions.The Psoriasis Area and Severity Index(PASI)score was 14.4.The Body Surface Area(BSA)score was 32%.The Physician's Global Assessment(PGA)score of psoriasis was 4.The patient was diagnosed with‘Plaque psoriasis’and treated with a variety of drugs,with poor clinical response.In addition,he has been on hemodialysis for uremia for the last 7 years.Treatment:The patient's rash improved a lot after subcutaneous injection with Ixekizumab,with no adverse effects seen during follow-ups.
