模拟培训在超声引导下股静脉穿刺置管教学中的应用效果 被引量:1

摘要 目的探讨模拟培训在超声引导下股静脉穿刺置管教学中的应用效果。方法选择2020年9月至2021年9月在陕西省肿瘤医院超声科进修学习的24名超声科医师为研究对象,通过抽签形式将其随机分为对照组(12名)和试验组(12名)。对照组采用传统教学方式,试验组采用传统教学联合模拟培训。比较两组的培训效果。结果两组医师的穿刺静脉回血时间比较,差异具有统计学意义(F=170.1,P<0.01)。在第1次至第14次操作,试验组每次操作的穿刺静脉回血时间均短于对照组;第15次至第20次操作,两组的穿刺静脉回血时间趋于相近。两组医师的自信心评分比较,差异具有统计学意义(F=144.8,P<0.01)。在第1次至第14次操作,试验组每次操作后的自信心评分均高于对照组;第15次至第20次操作,两组的自信心评分趋于相近。在最后3次操作中,两组医师的一次穿刺成功率、总穿刺成功率、静脉回血时间、误穿动脉率及局部血肿形成率比较,差异无统计学意义(P>0.05)。结论模拟培训可有效缩短医师超声引导下股静脉穿刺置管的学习曲线,使医师更快地掌握并熟练超声引导下股静脉穿刺置管技术。 Objective To explore the application effect of simulation training in the teaching of ultrasound-guided femoral vein catheterization.Methods From September 2020 to September 2021,24 ultrasound doctors who studied in the ultrasound department of Shaanxi Provincial Cancer Hospital were selected as the research objects.The doctors were randomly divided into control group(12 cases)and experimental group(12 cases)by drawing lots.The control group adopted traditional teaching method,and the experimental group adopted traditional teaching combined with simulation training.The training effects of the two groups were compared.Results There was a statistically significant difference in the time of puncture venous return between the two groups(F=170.1,P<0.01).In the 1st to the 14th operation,the puncture vein blood return time of each operation in the experimental group were shorter than those in the control group;from the 15th to the 20th operation,the puncture vein blood return time of the two groups tended to be similar.There was a statistically significant difference in the self-confidence score between the two groups(F=144.8,P<0.01).From the 1st to the 14th operation,the self-confidence scores of the experimental group after each operation were higher than those of the control group;from the 15th to the 20th operation,the self-confidence scores of the two groups tended to be similar.In the last 3 operations,there was no significant difference in the success rate of one puncture,the total success rate of puncture,the time of venous return,the rate of mistaken artery puncture and the rate of local hematoma formation between the two groups(P>0.05).Conclusion The simulation training can effectively shorten the learning curve of ultrasound-guided femoral vein catheterization,and enable doctors to quickly master and become proficient in the ultrasound-guided femoral vein catheterization technology.
