腹腔镜胆囊切除术治疗胆囊结石的临床效果及安全性分析

Analysis on Clinical Effect and Safety of Laparoscopic Cholecystectomy in Treatment of Cholecystolithiasis

目的:探讨腹腔镜胆囊切除术治疗胆囊结石的临床效果及安全性。方法:选取2021年1月—2022年12月于南通市通州区第八人民医院普外科行手术治疗的胆囊结石患者80例作为研究对象,采用抽签法随机分为对照组和观察组,各40例。对照组实施传统开腹手术,观察组实施腹腔镜胆囊切除术。比较两组治疗效果及安全性。结果:观察组手术时间、肠功能恢复时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.001)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.013)。术前,两组降钙素原(PCT)、超敏C反应蛋白(hs-CRP)水平比较,差异无统计学意义(P>0.05);术后,观察组PCT、hs-CRP水平均低于对照组,差异有统计学意义(P<0.001)。结论:腹腔镜胆囊切除术治疗胆囊结石的临床效果显著,可缩短手术时间,减少术中出血量,加速术后恢复,降低炎性反应,且安全性良好。

Objective:To investigate the clinical effect and safety of laparoscopic cholecystectomy in the treatment of cholecystolithiasis.Methods:A total of 80 patients with cholecystolithiasis undergoing surgical treatment in the General Surgery Department of Tongzhou District Eighth People's Hospital of Nantong City from January 2021 to December 2022 were selected as the study subjects.They were divided into observation group and control group by random lottery method,with 40 patients in each group.The control group underwent traditional open surgery.The observation group underwent laparoscopic cholecystectomy.Treatment effects and safety of the two groups were compared.Results:The operation time,intestinal function recovery time and hospital stay length in the observation group were shorter than those in the control group,and the intraoperative blood loss in the observation group was less than that in the control group,and the difference was statistically significant(P<0.001).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.013).Before operation,there was no significant difference in the levels of procalcitonin(PCT)and hypersensitive C-reactive protein(hs-CRP)between the two groups(P>0.05).After operation,the levels of PCT and hs-CRP in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001).Conclusion:Laparoscopic cholecystectomy has significant clinical effect in the treatment of cholecystolithiasis,which can shorten the operation time,reduce the intraoperative blood loss,accelerate postoperative recovery,reduce inflammatory reaction,and have good safety.
