急性心肌梗死患者术后药物康复期间应用下肢被动运动联合互动达标模式的效果观察

Effect of Lower Limb Passive Exercise Combined with Interactive Standard Mode in Patients with Acute Myocardial Infarction During Postoperative Drug Rehabilitation

目的:探讨下肢被动运动联合互动达标模式对急性心肌梗死经皮冠状动脉介入(PCI)术后患者服药依从性的影响。方法:将我院2022年1月—2023年2月期间收治的90例急性心肌梗死PCI术后患者作为观察对象,按照随机数字表法分为对照组(45例,给予常规护理)和观察组(45例,在对照组基础上给予下肢被动运动联合互动达标模式护理),观察两组心肌损伤标志物[脑钠肽(BNP)、肌酸激酶(CK)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CKMB)、心肌肌钙蛋白I(cTnI)]、心功能[左心室舒张末期容积(LVVd)、左心室短轴收缩率(FS)、左心室收缩末期容积(LVVs)、左心室射血分数(LVEF)]、服药依从性及心理状态。结果:出院时,观察组CK、CK-MB、BNP、LDH、cTnI低于对照组(P<0.05);观察组LVEF、FS值高于对照组,LVVd、LVVs值低于对照组(P<0.05)。干预1个月后,观察组服药依从性评分、SAS评分、SDS评分低于对照组(P<0.05)。结论:下肢被动运动联合互动达标模式护理可缓解急性心肌梗死PCI术后患者心肌损伤,改善心功能,提高用药依从性,缓解不良情绪。

Objective:To explore the effect of passive movement of lower limbs combined with interactive standard mode on medication compliance of patients with acute myocardial infarction after percutaneous coronary intervention(PCI).Methods:A total of 90 patients with acute myocardial infarction after PCI admitted to our hospital from January 2022 to February 2023 were selected as observation objects.According to the random number table method,they were divided into control group(45 patients,given routine nursing)and observation group(45 patients,given lower limb passive exercise combined with interactive standard mode nursing on the basis of the control group).The myocardial injury markers[brain natriuretic peptide(BNP),creatine kinase(CK),lactate dehydrogenase(LDH),creatine kinase isoenzyme(CKMB),cardiac troponin I(cTnI)],cardiac function[left ventricular end-diastolic volume(LVVd),left ventricular short-axis contraction rate(FS),left ventricular end-systolic volume(LVVs),left ventricular ejection fraction(LVEF)],medication compliance and psychological status were observed in the two groups.Results:At discharge,CK,CK-MB,BNP,LDH and cTnI in the observation group were lower than those in the control group(P<0.05);the LVEF and FS values of the observation group were higher than those of the control group,and the LVVd and LVVs values were lower than those of the control group(P<0.05).After 1 month of intervention,the scores of medication compliance,SAS and SDS in the observation group were lower than those in the control group(P<0.05).Conclusion:Lower limb passive exercise combined with interactive standard mode nursing can alleviate myocardial injury,improve cardiac function,improve medication compliance and relieve bad mood in patients with acute myocardial infarction after PCI.
