多西他赛联合顺铂治疗晚期非小细胞肺癌的临床疗效及对血清外泌体miR-21的影响

The Llinical Efficacy of Docetaxel Combined Cisplatin in theT reatment of Advanced Non-small Cell Lung Cancer and the Effect of Serum Exosome miR-21

目的:探讨多西他赛联合顺铂治疗晚期非小细胞肺癌的临床疗效及对血清外泌体miR-21水平的影响。方法:将南昌大学第四附属医院于2020年3月—2023年3月收治的96例晚期非小细胞肺癌患者分为对照组和观察组,每组48例。对照组予以培美曲塞+顺铂治疗,观察组则接受多西他赛+顺铂联合治疗,比较两组临床疗效、生存率、肿瘤无进展生存时间(PFS)、局部控制率及血清miR-21浓度。结果:观察组PFS高于对照组(P<0.05),而生存率、治疗有效率及局部控制率比较,差异无统计学意义(P>0.05);观察组治疗后miR-21浓度低于对照组,差异有统计学意义(P<0.05)。结论:非小细胞肺癌采用多西他赛与顺铂治疗可在保证治疗效果的基础上有效延长PFS,并降低miR-21水平,改善预后。

Objective:To investigate the clinical efficacy of docetaxel combined with cisplatin in the treatment of advanced nonsmall cell lung cancer and its effect on serum exosomal miR-21 level.Methods:A total of 96 patients with advanced non-small cell lung cancer admitted to the Fourth Affiliated Hospital of Nanchang University from March 2020 to March 2023 were divided into control group and observation group,with 48 patients in each group.The control group was treated with pemetrexed+cisplatin,while the observation group was treated with docetaxel+cisplatin.The clinical efficacy,survival rate,tumor progression-free survival time(PFS),local control rate and serum miR-21 concentration were compared between the two groups.Results:The PFS of the observation group was higher than that of the control group(P<0.05),there was no significant difference in survival rate,response rate and local control rate between the observation group and the control group(P>0.05).The concentration of miR-21 in the observation group was lower than that in the control group after treatment,and the difference was statistically significant(P<0.05).Conclusion:Docetaxel and cisplatin in the treatment of non-small cell lung cancer can effectively prolong PFS on the basis of ensuring the therapeutic effect,reduce the level of miR-21 and improve the prognosis.
