负荷量尿激酶原辅助PCI术治疗急性前壁心肌梗死的效果观察

Observation on the Effect of Load Prourokinase Assisted PCI in the Treatment of Acute Anterior Myocardial Infarction

目的:探讨负荷量尿激酶原联合PCI对急性前壁心肌梗死疗效观察。方法:选取我院2021年4月—2023年4月收治的240例急性前壁心肌梗死患者作为研究对象,分为观察组与对照组,每组120例。对照组患者采取急诊PCI联合常规药物治疗,观察组患者在对照组基础上PCI术前增加负荷量尿激酶原治疗。比较两组PCI介入术前后心肌梗死溶栓试验血流分级(TIMI),心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、肌酸激酶(CK)表达水平以及左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)心功能相关指标变化。结果:术后,观察组TIMI血流分级为2级的人数高于对照组(P<0.05);观察组cTnI、CK-MB、CK水平低于对照组(P<0.05);观察组LVEF高于对照组,观察组LVEDV低于对照组(P<0.05)。结论:负荷量尿激酶原联合PCI可改善急性前壁心肌梗死患者的TIMI分级,促进血流再通,改善心肌酶谱表达水平,提升患者心功能,值得临床应用。

Objective:To investigate the effect of load prourokinase combined with PCI on acute anterior myocardial infarction.Methods:A total of 240 patients with acute anterior myocardial infarction admitted to our hospital from April 2021 to April 2023 were selected as the research objects and divided into observation group and control group,with 120 patients in each group.The patients in the control group were treated with emergency PCI combined with conventional drugs,and the patients in the observation group were treated with loading dose of prourokinase before PCI on the basis of the control group.The changes of thrombolysis in myocardial infarction(TIMI),cardiac troponin I(cTnI),creatine kinase isoenzyme(CK-MB),creatine kinase(CK),left ventricular ejection fraction(LVEF)and left ventricular end diastolic volume(LVEDV)were compared between the two groups before and after PCI.Results:After surgery,the number of patients with TIMI blood flow grade 2 in the observation group was higher than that in the control group(P<0.05).The levels of cTnI,CK-MB,and CK in the observation group were lower than those in the control group(P<0.05).The LVEF of the observation group was higher than that of the control group,while the LVEDV of the observation group was lower than that of the control group(P<0.05).Conclusion:Load prourokinase combined with PCI can improve the TIMI classification of patients with acute anterior myocardial infarction,promote blood flow recanalization,improve the expression level of myocardial enzymes,and improve the cardiac function of patients,which is worthy of clinical application.
