唾液IL-5、IL-12、IL-13、IL-16水平与儿童乳牙龋齿活跃性的相关性

Correlation of salivary IL-5,IL-12,IL-13,IL-16 levels with deciduous teeth caries activity in children

摘要 目的探讨唾液白细胞介素(IL)-5、IL-12、IL-13、IL-16水平与儿童乳牙龋齿活跃性的相关性。方法选择2018年7月至2022年7月恩施土家族苗族自治州中心医院儿童牙科门诊接诊的142例患有乳牙龋齿的儿童(龋齿组)与51例无龋齿的儿童(对照组)为研究对象,根据龋齿活跃性(CAT)评分将龋齿组儿童分为低度活跃亚组(0~1.0分)39例、中度活跃亚组(1.5分)62例、重度活跃亚组(2.0~3.0分)41例。检测各组儿童唾液IL-5、IL-12、IL-13、IL-16水平,比较组间唾液IL-5、IL-12、IL-13、IL-16水平的差异;采用Spearman秩相关系数描述唾液IL-5、IL-12、IL-13、IL-16水平与CAT之间的相关性,以受试者工作特征(ROC)曲线分析唾液IL-5、IL-12、IL-13、IL-16水平鉴别重度CAT的价值。结果龋齿组唾液IL-5、IL-12、IL-13、IL-16水平均显著高于对照组,差异均有统计学意义(t值分别为23.662、20.822、20.358、37.689,P<0.01)。在低度活跃亚组、中度活跃亚组、重度活跃亚组中,唾液IL-5、IL-12、IL-13、IL-16水平比较差异均有统计学意义(F值分别为131.578、181.583、62.447、142.475,P<0.01);重度活跃亚组唾液IL-5、IL-12、IL-13、IL-16水平均高于中度活跃亚组(t值分别为5.356、9.373、7.284、9.538)和低度活跃亚组(t值分别为23.032、32.494、19.568、52.548),差异均有统计学意义(P<0.01);中度活跃亚组唾液IL-5、IL-12、IL-13、IL-16水平均高于低度活跃亚组(t值分别为10.368、11.177、3.137、6.360),差异均有统计学意义(P<0.05)。Spearman秩相关系数分析显示,唾液IL-5、IL-12、IL-13、IL-16水平与CAT均呈正相关(r值分别为0.477、0.268、0.293、0.357,P<0.01)。ROC曲线分析显示,唾液IL-5、IL-12、IL-13、IL-16鉴别重度CAT的最佳截断值分别为9.02pg/mL、2.15pg/mL、20.41pg/mL、369.75pg/mL,曲线下面积分别为0.691、0.773、0.680、0.727,唾液IL-5、IL-12、IL-13、IL-16联合鉴别重度CAT的曲线下面积为0.870,均高于唾液IL-5、IL-12、IL-13、IL-16单独鉴别方法(Z值分别为3.888、2.474、4.087、3.237,P<0.05)。结论患有乳牙龋齿儿童唾液IL-5、IL-12、IL-13、IL-16水平均明显增高,且与儿童乳牙龋齿严重程度有关。 Objective To investigate the correlation between the levels of salivary interleukin(IL)-5,IL-12,IL-13,IL-16 and caries activity of deciduous teeth in children.Methods A total of 142 children with deciduous tooth caries(caries group)and 51 children without caries(control group)who were admitted to the dental clinic of the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from July 2018 to July 2022 were selected as the study objects.According to the caries activity(CAT)score,they were divided into low active subgroup(0-1.0 points)39 cases,moderate active subgroup(1.5 points)62 cases,severely active subgroup(2.0-3.0 points)41 cases.The levels of IL-5,IL-12,IL-13 and IL-16 in saliva of all groups were detected,and the differences of IL-5,IL-12,IL-13 and IL-16 in saliva between groups were compared.Spearman rank correlation coefficient described the correlation between salivary IL-5,IL-12,IL-13,IL-16 levels and CAT.Receiver operating characteristic curve(ROC)was used to analyze the value of the salivary IL-5,IL-12,IL-13,IL-16 levels in identifying severe CAT.Results The levels of IL-5,IL-12,IL-13 and IL-16 in the caries group were higher than those in the control group,and the differences were statistically significant(t=23.662,20.822,20.358 and 37.689,respectively,P<0.01).There were significant differences in the levels of salivary IL-5,IL-12,IL-13 and IL-16 in the low active subgroup,the moderate active subgroup and the severe active subgroup(F=131.578,181.583,62.447 and 142.475,respectively,P<0.01).The levels of IL-5,IL-12,IL-13 and IL-16 in severe active subgroup were higher than those in the moderate active subgroup(t=5.356,9.373,7.284 and 9.538,respectively)and low active subgroup(t=23.032,32.494,19.568 and 52.548,respectively),and the differences were statistically significant(P<0.01).The levels of IL-5,IL-12,IL-13 and IL-16 in moderate active subgroup were higher than those in the low active subgroup(t=10.368,l1.177,3.137 and 6.360,respectively),and the difference were statistically significant(P<0.05).Spearman correlation coefficient analysis showed that the levels of IL-5,IL-12,IL-13,IL-16 were positively correlated with CAT(r=0.477,0.268,0.293 and 0.357,respectively,P<0.01).The ROC curve showed that the best cut-off values of saliva IL-5,IL-12,IL-13 and IL-16 to identify severe CAT was 9.02pg/mL,2.15pg/mL,20.4lpg/mL and 369.75pg/mL,respectively,and the area under the curve was 0.691,0.773,0.680 and 0.727,respectively.The area under the curve of saliva IL-5,IL-12,IL-13 and IL-16 combined to identify severe CAT was 0.870,which were higher than that of salivary IL=5,IL-12,IL-13 and IL-16 single identification(z=3.888,2.474,4.087 and 3.237,respectively,P<0.05).Conclusion The levels of IL-5,IL-12,IL-13 and IL-16 in saliva of children with deciduous teeth caries are significantly increased,which is related to the severity of deciduous teeth caries in children.
