卵巢癌患者术后心理弹性相关影响因素及护理对策分析

Analysis of Influencing Factors and Nursing Strategies for Postoperative Psychological Resilience of Ovarian Cancer Patients

目的探讨卵巢癌患者术后心理弹性相关影响因素,为通过针对性护理提高患者心理弹性水平提供参考。方法纳入2022年1月至12月福建省肿瘤医院114例卵巢癌手术患者为研究对象,以修订的中文版心理弹性量表(CD-RISC)对患者的心理弹性进行评估,以医院焦虑和抑郁量表(HADS)评估患者心理状态,另以自行设计的《一般情况调查表》收集患者人口学资料。Spearman线性分析CD-RISC与HADS评分的相关性,多因素logistic回归分析卵巢癌患者心理弹性影响因素。结果Spearman线性分析显示,卵巢癌患者CD-RISC与HADS各维度得分及总得分均呈负相关(均P<0.05)。多因素logistic回归分析显示,年龄、文化、婚姻、家庭收入、对卵巢癌了解程度、化疗疗程、医疗付费方式是卵巢癌患者心理弹性的独立影响因素。结论心理弹性可以负向预测卵巢癌患者的焦虑抑郁情绪,低龄、低文化、缺少伴侣、对卵巢癌不了解、化疗时间短、自付医疗费用的群体是心理弹性差的高发群体,应加强关注。针对个体情况予以针对性心理干预和疏导,以提高心理弹性水平,有助于降低患者焦虑抑郁、提高术后生活质量。

Objective To explore the influencing factors of postoperative psychological resilience in patients with ovarian cancer(OC),and to provide reference for improving patients'psychological resilience level through targeted nursing.Methods A total of 114 OC patients who met the standards in Fujian Cancer Hospital from January to December 2022 were selected as the research subjects.The revised Chinese version of the Connor-Davidson Resilience Scale(CD-RISC)was used to evaluate the patients'psychological resilience,and the Hospital Anxiety and Depression Scale(HADS)was used to evaluate the patients'psychological status.Additionally,a self-designed“General Situation Questionnaire”was used to collect demographic data from patients.Spearman linear analysis of the correlation between CD-RISC and HADS scores,and multiple logistic regression analysis of the influencing factors of psychological resilience in OC patients.Result Spearman's linear analysis showed that CD-RISC was negatively correlated with the scores of all dimensions and the total score of HADS in OC patients(all P<0.05).Multiple logistic regression analysis showed that age,culture,marriage,family income,knowledge of OC,chemotherapy course,and medical payment method were independent influences on psychological resilience in OC patients.Conclusions Psychological resilience can negatively predict the anxiety and depression of OC patients.The group with low age,low education,lack of partner,lack of understanding of OC,short chemotherapy time,and self paid medical expenses is a high-risk group with poor psychological resilience,and should be given more attention.Targeted psychological intervention and counseling should be given to individual situations to improve the level of psychological resilience,which can help reduce anxiety and depression in patients Improve postoperative quality of life.
