中药生大黄联合芒硝外敷对急性胰腺炎患者腹胀及舒适度的干预效果评价

Evaluation of the Intervention Effect of External Application of Traditional Chinese Medicine Raw Rhei Radix Et Rhizoma Combined with Natrii Sulfas on Abdominal Distension and Comfort in Patients with Acute Pancreatitis

目的探究中药生大黄联合芒硝外敷对急性胰腺炎患者腹胀及舒适度的干预效果。方法随机数字表法将福建医科大学附属第一医院2019年1月至2021年12月消化内科就诊的60例急性胰腺炎患者分为对照组(30例)与研究组(30例)。对照组接受常规西医干预,研究组在此基础上接受中药生大黄联合芒硝外敷。比较两组腹胀缓解时间、首次排便时间、肠鸣音恢复时间、舒适度、炎症指标、疼痛程度、胃肠功能评分、依从性、满意度。结果研究组腹胀缓解时间、首次排便时间及肠鸣音恢复时间比对照组短;研究组干预后精神、心理、生理维度评分比对照组高;研究组干预后C反应蛋白、降钙素原比对照组低;研究组干预后6、12、24、48 h的疼痛程度评分比对照组低;研究组干预后胃肠功能评分比对照组低;研究组依从性和满意度评分均比对照组高,差异均有统计学意义(均P<0.05)。结论中药生大黄联合芒硝外敷可明显缓解急性胰腺炎患者腹胀,提升其舒适度。

Objective To investigate the effect of external application of traditional Chinese medicine raw Rhei Radix Et Rhizoma combined with Natrii Sulfas on abdominal distension and comfort level of patients with acute pancreatitis.Methods A total of 60 patients with acute pancreatitis attending the Department of Gastroenterology of the First Affiliated Hospital of Fujian Medical University from January 2019 to December 2021 were divided into the control group(30 cases)and the study group(30 cases)by randomized numerical table method.The control group received conventional Western medical intervention,and the study group received external application of traditional Chinese medicine raw Rhei Radix Et Rhizoma combined with Natrii Sulfas on this basis.The two groups were compared in terms of time to relief of abdominal distension,time to first defecation,time to recovery of bowel sounds,comfort,inflammatory indicators,pain level,gastrointestinal function score,compliance,and satisfaction.Results The time to relief of bloating,time to first bowel movement and time to recovery of bowel sounds were shorter in the study group than in the control group;the scores of spiritual,psychological and physiological dimensions were higher in the study group than in the control group after the intervention;the C-reactive protein and calcitonin were lower in the study group than in the control group after the intervention;the scores of the degree of pain in the study group were lower in the study group than in the control group after the intervention at 6,12,24,48 h;the scores of the gastrointestinal function in the study group were lower in the study group than in the control group;the scores of Compliance and satisfaction scores were higher than those of the control group,and the differences were statistically significant(all P<0.05).Conclusions External application of traditional Chinese medicine raw Rhei Radix Et Rhizoma combined with Natrii Sulfas can significantly relieve abdominal distension and enhance the comfort level of patients with acute pancreatitis.
