基层卫生院药房处方调剂差错原因分析及防范对策

Analysis of the Causes and Preventive Measures for Prescription Dispensing Errors in Primary Health Hospital Pharmacy

目的探究基层卫生院药房处方调剂差错的原因,并提出相应的防范对策。方法将本基层卫生院药房在2023年1月开展实施处方调剂差错原因分析后的防范管理设为研究组,与2023年1月前开展的药房处方常规管理(对照组)进行对比,两组各选择500张处方单进行观察,分析处方调剂差错的原因并对比两组管理后的效果。结果药房处方调剂差错发生的原因包括人为因素、药品数量错误、药品位置摆放不合理等,研究组管理后的处方调剂差错发生率低于对照组,差异有统计学意义(P<0.05);研究组药房工作人员满意度评分、综合素质考核评分、药房管理质量评分均高于对照组,差异有统计学意义(P<0.05)。结论基层卫生院药房管理的过程中应不断的总结和归纳处方调剂出现差错的原因,并使用一系列的防范对策最大程度上减少差错的发生,保证处方的准确性,提高药房管理的质量水平和临床中用药的安全性。

Objective To explore the causes of prescription dispensing errors in grassroots health center pharmacies and propose corresponding preventive measures.Methods The prevention and management of prescription dispensing errors in the pharmacy of our grassroots health center were established as a research group in January 2023,and compared with the routine management of prescriptions in the pharmacy before January 2023(reference group).Each group selected 500 prescription forms for observation,analyzed the reasons for prescription dispensing errors,and compared the effectiveness of the two groups'management.Results The causes of prescription dispensing errors in pharmacies include human factors,incorrect drug quantity,and unreasonable drug placement.The incidence of prescription dispensing errors in the study group after management was lower than that in the reference group,and the difference was statistically significant(P<0.05).The satisfaction scores,comprehensive quality assessment scores,and pharmacy management quality scores of the research group's pharmacy staff were higher than those of the reference group,with statistically significant differences(P<0.05).Conclusions In the process of pharmacy management in grassroots health centers,it is necessary to continuously summarize and summarize the reasons for errors in prescription dispensing,and use a series of preventive measures to minimize the occurrence of errors,ensure the accuracy of prescriptions,improve the quality level of pharmacy management,and ensure the safety of medication in clinical practice.
