CPM锻炼仪联合早期量化康复训练对股骨转子间骨折患者术后髋关节功能康复效果的影响

Effect of CPM Exercise Instrument Combined with Early Quantified Rehabilitation Training on Postoperative Hip Functional Rehabilitation in Patients with Intertrochanteric Fracture of Femur

摘要 目的分析持续被动运动(CPM)锻炼仪联合早期量化康复训练对股骨转子间骨折(IFF)患者术后髋关节功能康复效果。方法选取我院IFF患者102例(2020年8月至2021年10月),根据治疗方案的不同分为对照组(n=51)和观察组(n=51)。对照组采取早期量化康复训练,观察组基于对照组采用CPM锻炼仪干预,对比两组髋关节功能[髋关节评分量表(HHS)]、关节活动度、肌力水平、疼痛程度[视觉模拟量表(VAS)]及并发症发生情况。结果治疗后,观察组HHS评分、关节活动度高于对照组(P<0.05);治疗后,两组伸直肌群肌力、屈曲肌群肌力比较,观察组高于对照组(P<0.05);治疗后,观察组住院时间、骨折愈合时间、下床运动时间均短于对照组(P<0.05);治疗后,两组VAS评分比较,观察组低于对照组(P<0.05);治疗后,观察组并发症发生率3.92%低于对照组15.69%(P<0.05)。结论CPM锻炼仪联合早期量化康复训练可改善IFF患者术后髋关节功能,能增强肌力水平,既减轻了手术部位的疼痛程度,又降低了并发症发生率,从而减速髋关节的康复进程。 Objective To analyze the effect of continuous passive motion(CPM)exercise instrument combined with early quantitative rehabilitation training on hip function rehabilitation of patients with intertrochanteric fracture(IFF)after operation.Methods A total of 102 IFF patients(from August 2020 to October 2021)in our hospital were selected and divided into control group(n=51)and observation group(n=51)according to different treatment regimens.The control group received early quantitative rehabilitation training,and the observation group received CPM exercise instrument based on the control group.Hip function[Harris Hip Score(HHS)],joint motion,muscle strength level,pain degree[Visual Analogue Scale(VAS)]and complications were compared between the two groups.Results After treatment,HHS score and range of motion in observation group were higher than those in control group(P<0.05).After treatment,the muscle strength of the stretching muscle and flexor muscle of the observation group was higher than that of the control group(P<0.05).After treatment,the hospitalization time,fracture healing time and getting out of bed time in the observation group were shorter than those in the control group(P<0.05).The VAS score of the observation group was lower than that of the control group after treatment(P<0.05).After treatment,the complication rate of observation group was 3.92%lower than control group 15.69%(P<0.05).Conclusions CPM exercise instrument combined with early quantitative rehabilitation training can improve hip joint function and enhance muscle strength in IFF patients after surgery,which can reduce the degree of pain at the surgical site and reduce the incidence of complications,thus slowing down the process of hip joint rehabilitation.
