摘要
目的探讨血小板与淋巴细胞比值(PLR)联合中性粒细胞与淋巴细胞比值(NLR)在消化系统恶性肿瘤中的诊断价值。方法选取100例消化系统恶性肿瘤患者作为研究组,根据肿瘤类型分为胃癌组、胰腺癌组和结直肠癌组,根据有无转移将研究组患者分为转移组和未转移组。选取同期体检的50例健康者作为对照组。比较消化系统恶性肿瘤患者以及健康体检者间的PLR、NLR水平。采用受试者工作曲线(ROC)分析PLR与NLR诊断消化系统恶性肿瘤的临床意义。结果胃癌组、胰腺癌组、结直肠癌组等消化系统恶性肿瘤患者PLR、NLR水平明显高于对照组(P<0.05),胃癌组、胰腺癌组、结直肠癌组间PLR、NLR水平比较差异无统计学意义。消化系统恶性肿瘤转移患者其PLR、NLR水平明显高于非转移组及对照组(P<0.05)。PLR对消化系统恶性肿瘤的曲线下面积AUC为0.841,95%CI(0.912~0.971),截断值为146.87,敏感度、特异性为68.90%、90.00%。NLR对消化系统恶性肿瘤的曲线下面积AUC为0.780,95%CI(0.713,0.847),截断值为2.21,敏感度、特异性为51.10%、98.90%。联合检测对消化系统恶性肿瘤的曲线下面积AUC为0.848,95%CI(0.920~0.976),敏感度、特异性为81.10%、92.20%。结论PLR与NLR在消化系统恶性肿瘤患者中具有较高的表达水平,并与是否转移有一定相关性,两者联合检测在诊断消化系统恶性肿瘤方面有较高的临床价值。
Objective To investigate the diagnostic value of platelet lymphocyte ratio(PLR)combined with neutrophil lymphocyte ratio(NLR)in digestive system malignant tumors.Methods One hundred patients with malignant tumors of digestive system were selected as the study group,which were divided into the gastric cancer group,the pancreatic cancer group and the colorectal cancer group according to the type of tumors.The study group patients were divided into the metastasis group and non-metastasis group according to metastasis.The levels of PLR and NLR were compared between patients with malignant tumors of digestive system and healthy people.The clinical significance of PLR and NLR in the diagnosis of gastrointestinal malignancies was analyzed with receiver operating curve(ROC).Results The levels of PLR and NLR of patients in the gastric cancer group,the pancreatic cancer group and the colorectal cancer group were significantly higher than those in the control group(P<0.05);there were no significant difference in PLR and NLR levels among the gastric cancer group,the pancreatic cancer group and the colorectal cancer group(P>0.05).The levels of PLR and NLR in patients with gastrointestinal malignant tumor metastasis were significantly higher than those in the non-metastatic group and the control group(P<0.05).The AUC of the area under the curve of PLR for digestive system malignant tumors was 0.841,95%CI(0.912~0.971),the cutoff value was 146.87,and the sensitivity and specificity were 68.90%and 90.00%.The AUC of the area under the curve of NLR for digestive system malignant tumors was 0.780,95%CI(0.713~0.847),the cutoff value was 2.21,and the sensitivity and specificity were 51.10%and 98.90%.The area under the curve AUC of combined detection for digestive system malignant tumors was 0.848,95%CI(0.920,0.976),and the sensitivity and specificity were 81.10%and 92.20%.Conclusion There are high expression levels in PLR and NLR of patients with digestive system malignant tumors,and they are related to metastasis.There is high clinical value in combined detection of PLR and NLR for the diagnosis of digestive system malignant tumors.
