镜像疗法在脑梗死合并吞咽功能障碍患者中的临床应用效果分析

Effective Analysis Clinical Application of Mirror Image Therapy with Dysphagia after Cerebral Infarction

目的:探讨镜像治疗在脑梗死合并吞咽功能障碍患者康复中的应用与效果。方法:2022年1月—2023年6月,选取上海市浦东新区老年医院康复医学科接受治疗的脑梗死合并吞咽功能障碍患者90例作为研究对象,将其随机分为观察组和对照组,每组各45例。对照组给予患者常规吞咽功能障碍训练,观察组在对照组基础上给予患者镜像治疗。对比两组患者治疗前、治疗2周、治疗4周、治疗8周洼田饮水试验、改良曼恩吞咽能力评估量表评分、并发症发生率、治疗依从性情况。结果:治疗后两组患者的洼田饮水试验分级较治疗前明显降低,改良曼恩吞咽能力评估量表评分均较治疗前明显升高;与对照组患者相比,观察组患者的洼田饮水试验分级下降更明显,改良曼恩吞咽能力评估量表评分升高更为明显;观察组患者并发症发生率明显低于对照组;观察组患者治疗依从性评分明显优于对照组。结论:基于镜像神经元理论的镜像疗法联合常规吞咽功能障碍训练,可以明显改善脑梗死合并吞咽功能障碍患者的吞咽功能,并发症发生率较低,治疗依从性较好,值得在临床上推广和应用。

OBJECTIVE To explore the application and effect of mirror image therapy in the rehabilitation of patients with swallowing dysfunction after cerebral infarction.METHODS From January 2022 to June 2023,90 patients with cerebral infarction combined with swallowing dysfunction treated in the Rehabilitation Medicine Department of Shanghai Pudong New Area Geriatric Hospital were selected as the research subjects.They were randomly divided into observation and control groups,with 45 cases in each group.Control group:patients were given routine swallowing dysfunction training,observation group:patients were given mirror treatment on the basis of routine training in the control group.Before treatment,2 weeks,4 weeks and 8 weeks of treatment.RESULTS Compared with before treatment,the Watian drinking water test scores of the two groups of patients at each time point after treatment were lower than before treatment,and the MMASA scores were higher than before treatment;compared with the control group,the Watian drinking water test grade in the treatment group decreased more significantly at each time point after treatment,and the MMASA score increased more significantly at each time point.The differences in complications and adverse reactions between the two groups were also statistically significant.CONCLUSION Mirror image therapy combined with swallowing training based on mirror neuron theory can significantly improve the swallowing function of patients with swallowing dysfunction after cerebral infarction,Good compliance,which is worthy of clinical promotion and application.
