茵苓健脾退黄汤泡浴联合双歧杆菌活菌治疗新生儿黄疸的疗效观察

Observation on the therapeutic efficacy of bath with Yinling Jianpi Tuihuang decoction combined with bifidobacterium live bacteria in the treatment of neonatal jaundice

目的分析茵苓健脾退黄汤泡浴联合双歧杆菌活菌治疗新生儿黄疸的疗效。方法选取我院于2019年1月-2022年6月收治的新生儿黄疸患儿98例,随机分为蓝光照射组和中药泡浴组,各49例。比较2组疗效、新生儿黄疸消退时间及住院时间、中医症状积分变化、胆红素水平、免疫球蛋白水平、每日摄奶量、体质量增加情况和不良反应发生情况。结果中药泡浴组总有效率为97.96%,蓝光照射组总有效率为85.71%,中药泡浴组总有效率高于蓝光照射组(P<0.05);中药泡浴组新生儿黄疸消退时间及住院时间均低于蓝光照射组(P<0.05);治疗后,2组主症(身目俱黄)及次症(腹部胀满、精神疲倦、乳食不思、尿黄便结、发热)积分均较治疗前低(P<0.05),且中药泡浴组治疗后积分低于蓝光照射组(P<0.05);治疗后,2组TBiL和DBiL均较治疗前低(P<0.05),且中药泡浴组治疗后TBiL和DBiL均低于蓝光照射组(P<0.05);治疗后,2组IgA和IgG均较治疗前高(P<0.05),且中药泡浴组治疗后IgA和IgG均高于蓝光照射组(P<0.05);中药泡浴组平均每日摄奶量和体重增加情况均高于蓝光照射组(P<0.05);2组不良反应发生率比较无统计学意义(P>0.05)。结论茵苓健脾退黄汤联合双歧杆菌活菌及蓝光治疗可明显改善新生儿黄疸症状,恢复胆红素水平,提高机体免疫,值得临床应用。

Objective To analyze the efficacy of bath with Yinling Jianpi Tuihuang decoction combined with bifidobacterium live bacteria in the treatment of neonatal jaundice.Methods 98 children with neonatal jaundice admitted to Qiqihar hospital of Traditional Chinese Medicine from January 2019 to June 2022 were selected and randomly divided into the blue light irradiation group(routine intermittent blue light and bifidobacterium live bacteria treatment)and the traditional Chinese medicine bath group(on the basis of the treatment in the blue light irradiation group,Yinling Jianpi Tuihuang decoction bath was used for treatment),with 49 cases in each group.The efficacy,neonatal jaundice dissipated time and hospital stay,changes of traditional Chinese medicine(TCM)symptoms scores,levels of bilirubins,levels of immunoglobulins,daily milk intake,increase in body mass,and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the traditional Chinese medicine bath group(97.96%)was higher than that in the blue light irradiation group(85.71%)(P<0.05).Neonatal jaundice dissipated time and hospital stay in the traditional Chinese medicine bath group were shorter than those in the blue light irradiation group(P<0.05).After treatment,the scores of the primary symptoms(yellow skin and eyes)and the secondary symptoms(abdominal distension,mental fatigue,no thought of food and milk,yellow urine and constipation,fever)in the two groups decreased compared with before treatment(P<0.05),and the scores in the traditional Chinese medicine bath group after treatment were lower than those in the blue light irradiation group(P<0.05).After treatment,the levels of total bilirubin(TBiL)and direct bilirubin(DBiL)in the two groups were lower than those before treatment(P<0.05),and the traditional Chinese medicine bath group was lower than the blue light irradiation group(P<0.05).The levels of IgA and IgG in both groups after treatment were increased compared with before treatment(P<0.05),and the traditional Chinese medicine bath group was higher than the blue light irradiation group(P<0.05).The average daily milk intake and weight gain in the traditional Chinese mecicine bath group were higher than those in the blue light irradiation group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yinling Jianpi Tuihuang decoction combined with bifidobacterium live bacteria and blue light therapy can significantly improve the symptoms of neonatal jaundice,restore the bilirubins levels,and improve the body's immunity,which is worthy of clininical application.
