抗生素骨水泥联合载抗生素硫酸钙人工骨在慢性骨髓炎治疗中的应用效果评价

Evaluation of the application effect of antibiotic bone cement combined with antibiotic loaded calcium sulfate artificial bone in the treatment of chronic osteomyelitis

目的分析抗生素骨水泥联合载抗生素硫酸钙人工骨在慢性骨髓炎治疗中的应用效果。方法选取2020年2月-2021年5月在河北省沧州中西医结合医院治疗的120例慢性骨髓炎患者,随机分为对照组和研究组,各60例。对照组采用载抗生素硫酸钙人工骨治疗,研究组采用抗生素骨水泥、载抗生素硫酸钙人工骨结合治疗。检测2组肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-1(interleukin-1,IL-1)、骨形态发生蛋白-2(Bone morphogenetic protein-2,BMP-2)、血常规情况、临床相关因素、疼痛程度表达情况变化。结果治疗后,2组TNF-α、IL-1、白细胞计数(white blood cell,WBC)、中性粒细胞(NEU)、红细胞沉降率(ESR)、视觉模拟评分法(Visual Analogue Scale,VAS)评分表达下降,BMP-2水平上升,且研究组较对照组改善更明显(P<0.05);与对照组相比,研究组手术时间、伤口复原、伤口拆线时间、住院时间均降低(P<0.05);研究组治疗总有效率高于对照组(P<0.05)。结论抗生素骨水泥、载抗生素硫酸钙人工骨联合治疗患者的慢性骨髓炎获得较良好的治疗效果,能有效改善患者疼痛感,缓解机体炎性水平,减少不良因素出现概率,为临床治疗开拓了新形式。

Objective To analyze the application effect of antibiotic bone cement combined with antibiotic loaded calcium sulfate artificial bone in the treatment of chronic osteomyelitis.Methods 120 patients with chronic osteomyelitis treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province from February 2020 to May 2021 were selected and randomly divided into the control groups and the study groups,with 60 cases in each group.The control group was treated with antibiotic loaded calcium sulfate artificial bone,and the study group was treated with antibiotic bone cement combined with antibiotic loaded calcium sulfate artificial bone.Tumor necrosis factor-α(TNF-α),interleukin-1(IL-10)and bone morphogenetic protein-2(BMP-2)levels,clinical related factors and pain degree expression changes were detected,and blood rutine was tested in both groups.Results After treatment,in both groups,TNF-αand IL-1 levels,white blood cell(WBC)count,neutrophils(NEU)count,erythrocyte sedimentation rate(ESR)and visual analogue scale(VAS)scores decreased,and the level of BMP-2 increased,and the study group showed more significant improvement compared with the control group(P<0.05);Compared with the control group,the operation time,wound recovery time,time for taking out stitches,and hospital stay in the study group were all shortened(P<0.05);The total effective rate of treatment was higher in the study group than that in the control group(P<0.05).Conclusion The combination of antibiotic bone cement and antibiotic loaded calcium sulfate artificial bone has achieved the good therapeutic effects,which can effectively improve the pain,relieve the inflammatory level in the body,reduce the probability of occurrence of adverse factors,and open up a new form for clinical treatment for this disease.
