中医痴呆病位、病因、病机制论框架结构研究

Research on Theoretical Framework Structure of Disease Location,Etiology and Pathogenesis of Dementia in Traditional Chinese Medicine

痴呆通常指因髓减脑消、神机失用而导致的,以呆傻愚笨为表现的一种神志类疾病。由于其病因、病机复杂多变,辨证论治颇为困难。中医学对该病的认识颇为悠久。该文以中医理论为基石,通过对中医经典、官修、各学说及医家的理论进行系统梳理,并从病位、病因、病机3个方面分析、归纳和总结痴呆的中医理论框架。作者通过文献梳理总结得出,痴呆的基本病位在脑,涉及肝、心、脾、肺、肾等五脏;其主要病因包括禀赋不足、药邪损体、外伤外邪、七情内伤、正气亏虚、他病继发等6个方面;主要病机以气血逆乱、痰浊阻窍、热扰神明、瘀阻脑络为主。该文旨在建立清晰、明确的中医理论框架,以期开拓中医古籍文献研究的新思路,同时为痴呆中医药临床防治提供新依据。

Dementia is a common mental disease,which is mainly caused by the loss of marrow and brain,and the loss of power,and its main clinical manifestation is stupidity.Because of its complex and changeable etiology and pathogenesis,it is quite difficult to treat it according to syndrome differentiation.Traditional Chinese medicine has a long history of understanding this disease.Based on the traditional Chinese medicine theory,this paper systematically combs the classic Chinese medicine,official medical records,various theories and the theories of doctors,and analyzes,classifies and summarizes the theoretical framework of traditional Chinese medicine for dementia from the three aspects of disease location,etiology and pathogenesis.Through literature review,the author concluded that the basic location of dementia is in the brain,involving the liver,heart,spleen,lung,kidney;The main causes of the disease include six aspects,such as insufficient endowment,damage to the body caused by drugs,external injury,internal injury caused by seven emotions,deficiency of vital energy,and the secondary occurrence of other diseases;The main pathogenesis is the disorder of Qi and blood,phlegm obstructing the orifices,heat disturbing the spirit,and blood stasis blocking the brain collaterals.This paper aims to establish a clear and clear theoretical framework of traditional Chinese medicine,with a view to providing new ideas for the study of ancient Chinese medicine literature and the treatment of dementia with traditional Chinese medicine,and to contribute to the inheritance and development of traditional Chinese medicine theory.
