清热祛痰推拿法联合穴位贴敷在儿童肺炎中的应用

Application of Heat Clearing and Phlegm Removing Massage Combinedwith Acupoint Application in Children with Pneumonia

目的 探究清热祛痰推拿法联合穴位贴敷在儿童肺炎中的应用效果。方法 采用前瞻性研究方法,纳入2020年9月—2022年2月南阳市第一人民医院86例细菌性肺炎患儿,采用随机数字表法分为两组各43例。两组均给予常规干预,对照组应用清热祛痰推拿法干预,观察组应用清热祛痰推拿法联合穴位贴敷干预。比较两组临床疗效、临床症状(发热、湿啰音、咳痰、咳嗽)消失时间、炎症因子水平[白细胞计数(WBC)、C反应蛋白(CRP)]及气道炎症标志物水平[呼出气一氧化氮(FeNO)]、肺通气功能指标[呼气流量峰值(PEF)、第一秒用力呼气容积占预计值百分比(FEV_1%pred)、一秒率(FEV_1/FVC)]。结果 干预后,观察组总有效率高于对照组(P<0.05);发热、湿啰音、咳痰、咳嗽症状消失时间均短于对照组(P<0.05);干预后,两组WBC、CRP、FeNO水平均降低,观察组均低于对照组(P<0.05);两组PEF、FEV_1%pred、FEV_1/FVC均较干预前提高,观察组均高于对照组(P<0.05)。结论 清热祛痰推拿法联合穴位贴敷应用于儿童肺炎中可获得较好的临床疗效,能缩短患儿临床症状改善时间,减轻炎症反应,改善其肺通气功能。

Objective To explore the application effect of heat clearing and phlegm removing massage combined with acupoint application in children with pneumonia.Methods In this study,a prospective study method was used to screen 86 children of Nanyang First Peoples Hospital with bacterial pneumonia admitted from September 2020 to February 2022.They were divided in-to two groups by random number table method,43 cases in each group.Both groups were given routine intervention.The control group was treated with heat clearing and expectorant massage,and the observation group was treated with heat clearing and ex-pectorant massage combined with acupoint application.The clinical effects of the two groups were compared at the end of the in-tervention.The disappearance time of clinical symptoms(fever,wet rales,expectoration and cough)was compared between the two groups.Before and at the end of the intervention,the levels of inflammatory factors[white blood cell count(WBC),C reac-tive protein(CRP)] and airway inflammatory markers[exhaled nitric oxide(FeNO)]were compared between the two groups.The pulmonary ventilation function indexes[peak expiratory flow(PEF),the percentage of forced expiratory volume in the first second(FEV,%pred),and the one second rate(FEV,/FVC)] were compared between the two groups before and after the in-tervention.Results At the end of intervention,compared with the control group,the total effective rate of intervention in the ob-servation group was higher and the overall efficacy was better(P<0.05).Compared with the control group,the disappearance time of fever,moist rale,expectoration and cough in the observation group was shorter(P<0.05).At the end of the interven-tion,the levels of WBC,CRP and FeNO in the two groups were lower than those before the intervention,and the levels of WBC,CRP and FeNO in the observation group were lower than those in the control group(P<0.05).PEF,FEV,%pred and FEV,/FVC in the two groups were higher than those before intervention,and PEF,FEV,%pred and FEV,/FVC in the observation group were higher than those in the control group(P<0.05).Conclusion Heat clearing and phlegm removing massage combined with acupoint application in children with pneumonia can obtain better clinical effect,shorten the time for improvement of clinical symptoms,reduce inflammatory reaction and improve lung ventilation function.
