活血疏肝汤联合耳穴压豆治疗肝郁血虚型卒中后抑郁的临床疗效

Clinical effect of Huoxue Shugan Decoction(活血疏肝汤)Combined with Ear Acupoint Pressing Beans in the Treatment of Post-stroke Depression with Liver Stagnation and Blood Deficiency

目的 探讨活血疏肝汤联合耳穴压豆治疗肝郁血虚型卒中后抑郁的临床疗效。方法 选取2019年6月—2022年6月在茂名市中医院治疗的肝郁血虚型卒中后抑郁的60例患者作为研究对象,采用随机数字表法分为对照组(30例)和试验组(30例),两组均给予心理治疗、康复运动,对照组患者给予活血疏肝汤治疗,试验组给予活血疏肝汤联合耳穴压豆治疗,治疗21天后对比两组的治愈总有效率,汉密尔顿抑郁量表(HAMD)评分、神经功能缺失程度(NIHSS)评分、Barthel指数(BI)和日常生活能力(ADL)评分的前后变化。结果 试验组(86.67%)治愈总有效率大于对照组(63.33%)(P<0.05),治疗前两组的HAMD评分、NIHSS评分、BI评分和ADL评分进行比较(P>0.05);治疗后两组的HAMD评分和NIHSS评分均小于治疗前,试验组BI评分和ADL评分大于对照组,且试验组幅度变化均大于对照组,(P<0.05),试验组(3.33%)不良反应发生率小于对照组(10.00%)(P>0.05)。结论 活血疏肝汤联合耳穴压豆治疗肝郁血虚型卒中后抑郁效果显著,可以有效缓解患者紧张不安、消极、抑郁等负面情绪,显著提升患者的生活品质,减少不良反应的发生。

Objective To investigate the clinical effect of Huoxue Shugan Decoction combined with ear acupoint pressing beans in the treatment of post-stroke depression of liver stagnation and blood deficiency type.Method Selected 60 patients with post-stroke depression of liver stagnation and blood deficiency type treated in Maoming Hospital of Traditional Chinese Medicine from September 2020 to January 2022 as the research objects,and divided them into the control group(30 cases)and the experimen-tal group(30 cases)by random number table method),both groups were given psychotherapy and rehabilitation exercise,the control group was given Huoxue Shugan Decoction,and the experimental group was treated with Huoxue Shugan Jieyu Decoction combined with auricular point pressing beans.Before-and-after changes in Depression Inventory(HAMD)score,Neurological Impairment Degree(NIHSS)score,Barthel Index(BI),and Activities of Daily Living(ADL)score.Result The total effective rate of cure in the experimental group(86.67%)was higher than that in the control group(63.33%),(P<0.05).Before treatment,the HAMD score,NIHSS score,BI score and ADL score of the two groups were compared(P>0.05).The HAMD score and NIHSS score of the latter two groups were lower than those before treatment,the BI score and ADL score of the experi-mental group were greater than those of the control group,and the amplitude changes of the experimental group were greater than those of the control group,(P<0.05),and adverse reactions occurred in the experimental group(3.33%).The rate was lower than that of the control group(10.00%),(P>0.05).Conclusion Huoxue Shugan Decoction combined with ear acupoint pressing beans has a significant effect on post-stroke depression of liver stagnation and blood deficiency type.
