摘要
目的探究血清和糖皮质激素调节蛋白激酶3(SGK3)、盐诱导激酶1(SIK1)在宫颈癌组织中的表达及其对患者预后的预测价值。方法选取石家庄市妇幼保健院2017年5月至2020年5月期间手术治疗的150例宫颈癌患者,通过免疫组化染色法检测宫颈癌组织及癌旁组织中SGK3、SIK1的表达,另选取同期体检的150例健康女性作为对照组,对比分析两组血清SGK3、SIK1的表达差异。采用Pearson相关分析血清SGK3、SIK1表达相关性,进一步通过COX回归分析宫颈癌患者预后的影响因素,受试者工作特征(ROC)曲线分析血清SGK3、SIK1对宫颈癌患者预后的评估价值,Kaplan-Meier法分析宫颈癌组织SGK3、SIK1表达与患者预后的关系。结果与癌旁组织相比,宫颈癌组织SGK3阳性表达率升高、SIK1阳性表达率降低(P<0.05);与对照组相比,宫颈癌患者血清SGK3表达水平升高,SIK1表达水平降低,二者之间呈现负相关(r=-0.347,P<0.05);COX回归分析显示淋巴结转移、血清SGK3、SIK1表达为宫颈癌患者预后的影响因素(P<0.05)。血清SGK3预测宫颈癌患者预后的曲线下面积(AUC)为0.802,灵敏度为60.00%,特异度为89.09%;血清SIK1预测宫颈癌患者预后的AUC为0.737,灵敏度为75.00%,特异度为67.27%;二者联合的AUC为0.866,灵敏度为87.50%,特异度为71.82%。宫颈癌组织SGK3阴性表达患者生存率(85.71%)高于SGK3阳性表达患者生存率(65.96%),SIK1阳性表达患者生存率(81.36%)高于SIK1阴性表达患者生存率(63.74%)(P<0.05)。结论宫颈癌患者癌组织和血清中SGK3表达水平均上调、SIK1表达水平均下调,宫颈癌组织SGK3阴性表达、SIK1阳性表达患者生存率较高,血清SGK3、SIK1水平对评估宫颈癌患者预后具有一定临床价值。
Objective To investigate the expressions of serum and glucocorticoid-induced protein kinase 3(SGK3)and salt-inducible kinase 1(SIK1)in cervical cancer tissues,and their predictive value for the prognosis of patients.Methods The data of 150 patients with cervical cancer who underwent surgical treatment in Shijiazhuang Maternal and Child Health Hospital from May 2017 to May 2020 were using immunohistochemical staining to detect the expression of SGK3 and SIK1 in cervical cancer tissues and adjacent tissues.Besides,150 healthy women who accepted physical examination during the same period were selected as the control group.The expression differences of serum SGK3 and SIK1 between the two groups were analyzed.Pearson correlation analysis was performed to analyze the correlation of serum SGK3 and SIK1 expression,and the influencing factors of prognosis of cervical cancer patients were further analyzed by COX regression,receiver operating characteristic(ROC)curve was performed to analyze the evaluation value of serum SGK3 and SIK1 in the prognosis of cervical cancer patients,and Kaplan-Meier method was used to analyze the relationship between the expression of SGK3 and SIK1 in cervical cancer tissues and the prognosis of patients.Results Compared with the adjacent tissue,the positive expression rate of SGK3 in cervical cancer tissue was increased,and the positive expression rate of SIK1 was decreased(P<0.05).compared with the control group,the expression level of serum SGK3 in cervical cancer patients was increased,and the expression level of SIK1 was decreased,and there was a negative correlation between the two groups(r=-0.347,P<0.05).COX regression analysis showed that lymph node metastasis,serum SGK3 and SIK1 expressions were influencing factor for the prognosis of cervical cancer patients(P<0.05).The area under the curve(AUC)of serum SGK3 in predicting the prognosis of cervical cancer patients is 0.802,with a sensitivity of 60.00%and a specificity of 89.09%.The AUC of serum SIK1 in predicting the prognosis of cervical cancer patients is 0.737,with a sensitivity of 75.00%and a specificity of 67.27%;The combined AUC of the two is 0.866,with a sensitivity of 87.50%and a specificity of 71.82%.The survival rate of SGK3 negative expression patients(85.71%)in cervical cancer tissue was higher than that of SGK3 positive expression patients(65.96%),and the survival rate of SIK1 positive expression patients(81.36%)was higher than that of SIK1 negative expression patients(63.74%)(P<0.05).Conclusion The expression levels of SGK3 and SIK1 in cancer tissues and serum of patients with cervical cancer are both up-regulated and the expression level of SIK1 is down-regulated.Patients with negative expression of SGK3 and positive expression of SIK1 in cervical cancer tissue have a higher survival rate,and the levels of serum SGK3 and SIK1 have certain clinical value in evaluating the prognosis of cervical cancer patients.
