基于盆底检测结果的个性化治疗方案对产后盆底肌功能康复的影响

Effect of individualized treatment based on pelvic floor test results on postpartum pelvic floor muscle function rehabilitation

目的探讨基于盆底检测结果的个性化治疗方案对产后盆底功能恢复的影响。方法选取2021年1~12月于山东第一医科大学第一附属医院妇产科盆底诊治中心诊治的产后42 d盆底肌力为0-Ⅰ级、盆腔器官脱垂定量分期法(pelvic organ prolapsed quantitation,POP-Q)分期为0-Ⅱ期的46例产妇作为研究对象。按照患者意愿将其分为观察组(23例)和对照组(23例)。对照组按照盆底治疗仪设定的固定治疗方案进行15次的盆底治疗。观察组根据患者盆底检测结果以及每次治疗前再评估制定个性化的治疗方案进行15次的盆底治疗。比较两组治疗前后盆底肌力、POP-Q分期。结果治疗后两组盆底肌力较治疗前升高,且观察组盆底肌力高于对照组,差异有统计学意义(P<0.05)。治疗后两组POP-Q分期比较,差异无统计学意义(P>0.05)。两组治疗前后POP-Q分期比较,差异均有统计学意义(P<0.05)。结论治疗方案的选择对于改善及恢复产后盆底功能具有重要影响,基于盆底检测结果的个性化治疗方案对提高产后盆底肌功能的治疗效果较为显著,值得临床推广应用。

Objective To investigate the effect of individualized treatment based on pelvic floor test results on postpartum pelvic floor muscle function rehabilitation.Methods From January 2021 to December 2021,a total of 46 parturients with pelvic floor muscle strength of 0-Ⅰ,pelvic organ prolapsed quantitation(POP-Q)grading 0-Ⅱ,and 42 days after delivery were selected who visited the Pelvic Floor Diagnosis and Treatment Center of the First Affiliated Hospital of Shandong First Medical University.The patients were divided into observation group(23 cases)and control group(23 cases)according to their aspirations.The control group received 15 pelvic floor treatments according to the fixed treatment plan set by pelvic floor therapy instrument.The observation group received 15 pelvic floor treatments according to the results of pelvic floor testing and reassessment before each treatment.The pelvic floor muscle strength and POP-Q grading of the two groups were compared before and after treatments.Results The pelvic floor muscle strength of the two groups after treatment was higher than that before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,there was no significant difference in POP-Q grading between the two groups(P>0.05).There was a statistically significant difference in POP-Q staging between the two groups before and after treatment(P<0.05).Conclusion The choice of treatment plan plays an important role in improving and restoring pelvic floor function after delivery.The individualized treatment based on pelvic floor test results has a significant therapeutic effect on improving the function of postpartum pelvic floor muscle,which is worthy of promotion.
