壮腰合剂治疗寒湿痹阻型腰椎间盘突出症临床观察

Lumbar Disc Herniation Case with Cold-dampness Obstruction Syndrome Treated with Zhuangyao Mixture(壮腰合剂)

目的观察壮腰合剂改善寒湿痹阻型腰椎间盘突出症患者疼痛情况和腰腿部活动能力的临床疗效。方法选择2021年7月—2022年7月医院康复医学科门诊诊断治疗的120例寒湿痹阻型腰椎间盘突出症患者为研究对象,随机分为中药组和西药组,每组60例。西药组给予塞来昔布胶囊治疗,100 mg/次,2次/d口服;中药组给予中药经验方壮腰合剂治疗,50 mL/次,2次/d口服。5 d为1个疗程,两组疗程均为10 d,观察并比较两组患者治疗前后临床疗效、中医证候积分、视觉模拟评分(Visual analogue scale,VAS)、日本骨科协会评估治疗分数(Japanese orthopaedic associationscores,JOA)、Oswestry功能障碍指数问卷评分(ODI)、焦虑自评表(Selfrating anxiety scale,SAS)。结果两组患者治疗后,中药组有效率为93.33%(56/60)高于西药组的78.33%(47/60),差异有统计学意义(P<0.05);治疗后中药组中医证候积分、ODI、SAS、VAS均较西药组降低(P<0.05),差异具有统计学意义;治疗后中药组JOA评分高于西医组(P<0.05),差异具有统计学意义;结论壮腰合剂能明显改善寒湿痹阻型腰椎间盘突出症患者的疼痛及腰腿部活动能力,提高临床疗效。

Objective To observe the clinical effect of Zhuangyao Mixture(壮腰合剂)in improving the pain and the activity of the waist and legs of lumbar disc herniation patients with cold-dampness obstruction syndrome.Method From July 2021 to July 2022,120 patients with cold-dampness obstruction type of lumbar disc herniation diagnosed and treated in the outpatient depart-ment of rehabilitation medicine of the hospital were selected as the research objects.They were randomly divided into traditional Chinese medicine group and Western medicine group,with 60 cases in each group.The western medicine group was treated with celecoxib capsules,100 mg/time,twice a day.The traditional Chinese medicine group was treated with Zhuangyao Mixture,50 mL/time,twice a day.The course of treatment in both groups was 10 days.The clinical efficacy,TCM syndrome score,visual an-alogue scale(VAS),Japanese orthopaedic association(JOA)scores,Oswestry dysfunction index(ODI)questionnaire scale and self-rating anxiety scale(SAS)of two groups were observed and compared.Result After treatment,the effective rate of the tradi-tional Chinese medicine group was 90%,higher than that of the western medicine group(78.3%),and the difference was statisti-cally significant(P<0.05).After treatment,the TCM syndrome score,ODI,SAS and VAS of the traditional Chinese medicine group were lower than those of the western medicine group(P<0.05),and the difference was statistically significant.After treat-ment,the JOA score of the traditional Chinese medicine group was higher than that of the western medicine group(P<0.05)and the difference was statistically significant.Conclusion Zhuangyao Mixture can obviously improve the pain and the activity ability of the waist and legs of the patients with cold-dampness obstruction type of lumbar disc herniation,and improve the clinical effi-cacy.
