苍龟探穴针法联合温灸、刺络放血治疗慢性颈肩痛疗效观察

Clinical Observation of Canggui Tanxue Needling Combined with Moxibustion and Blood-letting Puncture Therapies in Treatment of Chronic Neck and Shoulder Pain

目的研究苍龟探穴针法联合温灸、刺络放血对慢性颈肩痛患者效果观察。方法2020年3月—2021年12月收治的68例慢性颈肩痛患者做回顾性分析,根据治疗方案的不同分为研究组30例和对照组38例,对照组给予温灸和刺络放血治疗,研究组联合苍龟探穴针法治疗。对比两组临床疗效;对比两组患者治疗前后中医证候积分、疼痛程度评分、肩关节活动评分、日常生活能力评分;对比两组患者治疗期间不良反应发生率。结果与对照组总有效率81.57%(31/38)相比,研究组总有效率96.66%(29/30)显著更高(P<0.05);治疗后,两组患者疲倦乏力、肩部肿胀、肢体困重和活动不便积分、视觉模拟疼痛(VAS)评分、肩关节活动(Melle)评分均显著降低(P<0.05);治疗后,研究组疲倦乏力、肩部肿胀、肢体困重和活动不便积分、VAS评分、Melle评分均显著低于对照组(P<0.05);治疗后,研究组日常生活能力量表(ADL)评分显著高于对照组(P<0.05);对照组不良反应发生率7.89%(3/38),与研究组6.66%(2/30)相比,差异不具有统计学意义(P>0.05)。结论苍龟探穴针法联合温灸、刺络放血对慢性颈肩痛患者的治疗效果较高,可有效改善患者的中医证候积分和临床症状,且安全性高。

Objective To study the clinical efficacy of Canggui Tanxue(green turtle probing its cave)needling combined with moxibustion and blood-letting puncture therapies in the treatment of chronic neck and shoulder pain.Methods Sixty-eight pa-tients with chronic neck and shoulder pain in the hospital from March 2020 to December 2021 were enrolled and classified into two groups according to different treatment methods.The control group(38 cases)received moxibustion and blood-letting punc-ture therapies.Based on this,the study group(30 cases)received Canggui Tanxue needling therapy.Then the comparison was conducted on TCM syndrome score,Visual Analogue Scale(VAS)score,Melle score(shoulder joint activity),Activity of Daily Life(ADL)score and adverse reaction rate were compared.Results The total efficacy rate was 96.66%(29/30)in the study group and was higher than 81.57%(31/38)in the control group,with statistical difference(P<0.05).After treatment,a reduc-tion in TCM syndrome scores including fatigue and weakness,shoulder swelling,limb drowsiness and loss of mobility,VAS score and Melle score,and the changes in the study group were more remarkably than those in control group,with statistical difference(P<0.05).ADL score was obviously decreased in both groups before treatment(P<0.05),while the ADL score in the study group was higher than that in the control group after treatment(P<0.05).The adverse reaction rate was 7.89%(3/38)in the control group,which showed no statistical difference compared with 6.66%(2/30)in the study group(P>0.05).Conclusion-Application of Canggui Tanxue needling combined with moxibustion and blood-letting puncture therapies in the treatment of chronic neck and shoulder pain can effectively ameliorate the TCM syndrome score and clinical symptoms,with high treatment safety.
