宣肺化痰、蠲饮平喘法治疗痰浊壅肺型慢性心力衰竭临床观察

Efficacy observation of method for dispersing lung qi and resolving phlegm,eliminating fluid-retention and relieving asthma on chronic heart failure with syndrome of turbid phlegm obstructing lung

目的:观察宣肺化痰、蠲饮平喘法治疗痰浊壅肺型慢性心力衰竭的临床疗效。方法:选取55例痰浊壅肺型冠心病慢性心力衰竭患者,按照随机数字表法分为治疗组28例与对照组27例。两组患者均予西医常规抗心力衰竭治疗和心脏康复治疗,治疗组在对照组基础上加用真武汤合葶苈大枣泄肺汤加减治疗,两组疗程均为2周。观察并比较两组临床疗效。结果:治疗后,治疗组临床总有效率96.43%高于对照组的81.48%(P﹤0.05)。两组6 min步行距离值及LVEF值均高于治疗前(P﹤0.05);两组中医证候积分值、NT-proBNP值及NYHA值均低于治疗前(P﹤0.05);治疗后以上各指标水平组间比较均有统计学意义(P﹤0.05)。结论:真武汤合葶苈大枣泄肺汤加减联合西药治疗痰浊壅肺型慢性心力衰竭患者临床疗效较好,可有效控制疾病发展,提高患者的心功能与运动能力。

Objective:To observe the clinical efficacy of method for dispersing lung qi and resolving phlegm,eliminating fluid-retention and relieving asthma on chronic heart failure with syndrome of turbid phlegm obstructing lung.Methods:55 cases of patients with chronic heart failure caused by coronary heart disease with syndrome of turbid phlegm obstructing lung were selected and divided into treatment group(28 cases)and control group(27 cases)by random number table.The patients in both groups were treated with routine western medicine for anti-heart failure and by cardiac rehabilitation.In addition,the treatment group was treated plus with Zhenwu decoction and modified Tingli Dazao Xiefei decoction on the basis of control group.The clinical effects of two groups were observed and compared.Results:After treatment,the total clinical effective rate of treatment group was 96.43%,which was higher than 81.48%of control group(P﹤0.05).In both groups,6-minute walking distance and LVEF were higher than those before treatment(P﹤0.05).In two groups,TCM syndrome score,NT-proBNP and NYHA were lower than those before treatment(P﹤0.05).Compared the above each indexe between two groups after treatment,the differences were statistically significant(P﹤0.05).Conclusion:The clinical efficacy of Zhenwu decoction and modified Tingli Dazao Xiefei decoction and western medicine on chronic heart failure with syndrome of turbid phlegm obstructing lung is significant,which can effectively control the development of the disease and improve the cardiac function and exercise ability of patients.
