加减归肾丸联合宫腔内夫精人工授精对不孕症(肾阳虚证)患者妊娠率影响的临床观察

Clinical Observation on the Pregnancy Rate of Patients with Infertility(Kidney Yang Deficiency Type)by Combining Intrauterine Insemination with Modified Guishen Pill(归肾丸)

目的探讨加减归肾丸联合宫腔内夫精人工授精(AIH)对不孕症(肾阳虚证)患者临床妊娠情况的影响。方法回顾性分析辽宁中医药大学附属医院在2019年1月—2022年11月期间行AIH的86例患者的病历资料,共152个周期。根据AIH术后不同用药方案,分为对照组(黄体酮软胶囊组)95个周期,治疗组(中药联合黄体酮软胶囊组)57个周期。比较两组患者临床疗效、妊娠率情况以及妊娠者AIH平均治疗周期数。结果治疗组肾阳虚证候总有效率显著高于对照组(92.11%VS 56.25%),差异有统计学意义(P<0.05)。治疗组临床妊娠周期率显著高于对照组(28.07%VS 14.73%),差异有统计学意义(P<0.05)。治疗组妊娠患者AIH平均治疗周期数少于对照组(1.50±0.83)VS(1.98±1.06),差异有统计学意义(P<0.05)。结论在AIH治疗过程中,使用加减归肾丸可以改善患者肾阳虚症状,减少AIH治疗周期数,提高不孕症(肾阳虚证)患者临床妊娠率。

Objective To explore the effect of combining artificial insemination of husband(AlH)with Modified Guishen Pill(归肾丸加减)on clinical pregnancy in patients with infertility(kidney Yang deficiency type).Methods The medical records of 86 patients who underwent AIH at the affiliated hospital of Liaoning university of traditional Chinese medicine during January 2019November 2022 were retrospectively analyzed for a total of 152 cycles.According to different medication regimens after AIH surgery,there were 95 cycles in the control group(progesterone soft capsule group)and 57 cycles in the treatment group(herbal medicine combined with progesterone soft capsule group).The clinical outcomes,pregnancy rates and the average number of AIH treatment cycles in pregnant patients were compared between the two groups.Results The total effective rate of kidney yang deficiency symptoms was significantly higher in the treatment group than in the control group(92.11%vs 56.25%),and the difference was statistically significant(P<0.05).The clinical pregnancy cycle rate in the treatment group was significantly higher than that in the control group(28.07%vs 14.73%),and the difference was statistically significant(P<0.05).The mean number of AIH treatment cycles in pregnant patients in the treatment group was less than that in the control group(1.50±0.83)vs(1.98±1.06),and the difference was statistically significant(P<0.05).Conclusion During the treatment of AIH,the use of Modified Guishen Pill can improve the symptoms of kidney Yang deficiency type in patients,reduce the numher of AIH treatment cycles,and improve the clinical pregnancy rate in patients with infertility(kidney Yang deficiency type).
