基于数据挖掘探究中药药浴治疗新生儿黄疸的用药规律

Exploring the medication rules of TCM medicine bath in the treatment of neonatal jaundice based on data mining

目的:基于数据挖掘总结中药药浴治疗新生儿黄疸的用药规律,为临床治疗与研究提供数据参考。方法:检索中国知网2002年1月1日-2022年1月1日发表的中药药浴治疗新生儿黄疸的相关文献,筛选符合纳入标准的文献并构建数据库,运用Excel 2019对其高频药物的四气、五味、归经及功效进行分析,运用IBM SPSS Modeler 18.0、IBM SPSS Statistics 21.0软件对其药物关联规则及聚类分析进行研究。结果:共纳入中药处方67首,中药77种,使用频数≥10的中药共16种,其中茵陈的使用频数最高。药性以寒性为主,药味以苦、甘、辛为主,归经多属肝经,药物功效以清热、利水渗湿为主。药物关联规则中高频药对以茵陈-栀子、茵陈-大黄、栀子-大黄为主。药物聚类分析得到了5个聚类。结论:临床上以湿热郁蒸型胎黄多见,中药药浴治疗新生儿黄疸多以清热利湿退黄的治法为主,方中药物以苦、寒药物居多,治疗过程中要注意顾护后天脾胃。

Objective:To summarize the medication rules of TCM medicine bath in the treatment of neonatal jaundice based on data mining,so as to provide data reference for clinical treatment and research.Methods:Relevant literature on the treatment of neonatal jaundice by TCM medicine baths in CNKI from January 1,2002 to January 1,2022 was searched,and the literature that met the inclusion criteria was screened and a database was constructed.Excel 2019 was used to analyze the high-frequency drugs,four properties,fiveO tastes,belonged meridians,and efficacy of the drugs.IBM SPSS Modeler 18.0 and IBM SPSS Statistics 21.0 software were used to study the drug association rules and cluster analysis.Results:A total of 67 prescriptions and 77 kinds of TCM medicine were included,and 16 kinds of TCM medicine were used more than 10 times,among which the frequency of Yinchen(Herba Artemisiae Scopariae)was the highest.The medicinal property was mainly cold,the medicinal tastes were mainly bitter,sweet and acrid,and most of the meridians belonged to the liver meridian.The efficacy of the drugs were mainly to clear away heat,diuresis and dampness.In the drug association rules,the high frequency drug pairs were mainly Yinchen-Zhizi(Fructus Gardeniae),Yinchen-Dahuang(Radix et Rhizoma Rhei),and Zhizi-Dahuang.Five clusters were obtained by drug cluster analysis.Conclusion:Clinically,neonatal jaundice is more common with damp-heat stagnation and steaming.The treatment of neonatal jaundice by TCM medicine bath is mainly based on the treatment of clearing away heat,eliminating dampness and relieving jaundice.The medicines in the prescription are mostly bitter and cold medicines.The spleen and stomach need to be taken care of during the treatment.
