温经祛痛汤联合脐灸治疗寒凝血瘀型原发性痛经的临床疗效

Clinical efficacy of the Wenjing Qutong decoction plus umbilical moxibustion on primary dysmenorrhea of cold coagulation and blood stasis type

目的:原发性痛经是一种临床常见妇科疾病,以经期或行经前后小腹疼痛、坠胀,伴腰酸或其他不适,甚至剧痛难以忍受,影响生活为主要表现,临床发病率逐渐增加,且病情反复,对患者的学习、工作及生活造成严重困扰。西医治疗本病以药物止痛治疗为主,能有效缓解疼痛,但不良反应较多,停药易反复,需要长期用药。本研究主要观察自拟温经祛痛汤联合脐灸治疗寒凝血瘀型原发性痛经患者的临床疗效。方法:将寒凝气滞血瘀型的原发性痛经患者(共80例)随机分为治疗组及对照组,每组40例;对照组给予西药布洛芬缓释胶囊,治疗组给予自拟温经祛痛汤联合脐灸治疗,连续治疗3个月经周期,观察对比两组试验对象治疗后的临床疗效。结果:两组治疗后血清前列腺素F2α、缩宫素水平较治疗前明显减少,差异具有统计学意义(P<0.05);前列腺素E2、β-内啡肽水平明显上升,差异具有统计学意义(P<0.05),且治疗组改善更显著,差异具有统计学意义(P<0.05);治疗组总有效率为92.5%(37/40),对照组为75.0%(30/40),组间比较,差异有统计学意义(P<0.05);随访6个月,治疗组疗效稳定,不易复发。结论:自拟温经祛痛汤联合脐灸治疗寒凝血瘀型原发性痛经能有效缓解患者腹痛及临床症状,改善前列腺素、缩宫素、β-内啡肽水平,调节内分泌系统,改善子宫血流量,疗效稳定且不容易复发,值得临床推广。

Objective:Primary dysmenorrhea is a common clinical gynecological disease,with menstrual or pre-and post-menstrual lower abdomen pain,swelling,accompanied by backache or other discomfort,and even severe pain is unbearable,affecting life as the main manifestations,its clinical incidence gradually increases,and the condition is repeated,causing serious troubles to the patient’s study,work and life.Western medicine treatment of this disease is mainly based on pain relief drug,which canO effectively relieve pain,but there are many adverse reactions,and it is easy to repeat and require long-term medication.The clinical efficacy of the Wenjing Qutong decoction(温经祛痛汤)plus umbilical moxibustion on primary dysmenorrhea of cold coagulation and blood stasis type is observed in this paper.Methods:A total of 80 patients with primary dysmenorrhea of the cold coagulation and blood stasis type were randomly divided into the treatment group and the control group,with 40 patients in each group.The control group was given ibuprofen sustained-release capsules,and the treatment group was given the Wenjing Qutong decoction plus umbilical moxibustion for 3 consecutive menstrual cycles.Results:After treatment,the levels of serum PGF2αand oxytocin in the two groups were significantly lower than those before treatment,and the difference was statistically significant(P<0.05).The levels of PGE2 andβ-EP were significantly increased,and the difference was statistically significant(P<0.05).The improvement in the treatment group was more significant,and the difference was statistically significant(P<0.05).The total efficiency was 92.5%(37/40)in the treatment group and 75.0%(30/40)in the control group,and the difference between the groups was statistically significant(P<0.05).After 6 months of followed-up,the curative effect in the treatment group was stable and recurrence was not easy.Conclusion:The Wenjing Qutong decoction plus umbilical moxibustion can effectively relieve abdominal pain and clinical symptoms of patients with primary dysmenorrhea of cold coagulation and blood stasis type.It can improve prostaglandin,oxytocin,β-endorphin levels,regulate the endocrine system,improve uterine blood flow,and the curative effect is stable and not easy recurrence.It is worthy of clinical promotion.
