sd-LDL及IMA与急性缺血性脑卒中的相关性分析

摘要 目的 探讨小而密低密度脂蛋白胆固醇(small dense low-density lipoprotein, sdLDL-C)及缺血修饰蛋白(ischemia modified albumin, IMA)与急性缺血性脑卒中相关性。方法 选取2021年1月至2021年5月于锦州医科大学附属第一医院确诊的101例急性脑卒中患者作为研究对象,另选取锦州医科大学附属第一医院同期健康体检者47例为对照组。比较两组患者的血清sdLDL-C、IMA等水平,分析其与病情严重程度(national institutes of health stroke scale, NIHSS)相关。结果 病例组的sdLDL-C及IMA表达水平高于对照组,且差异具有统计学意义,不同神经缺损程度患者sdLDL-C、IMA差异具有统计学意义。吸烟、高血压、糖尿病、sdLDL-C、IMA、低密度脂蛋白(low-density lipoprotein, LDL)是缺血性脑卒中危险因素,且LDL、sdLDL-C、IMA危险程度较高,根据ROC曲线,发现IMA、sdLDL-C联合检测效能更优。同时吸烟、糖尿病、LDL、IMA、sdLDL-C与NIHSS评分成正相关,sdLDL-C和IMA最为密切。结论 sdLDL-C是缺血性脑卒中重要危险因素,sdLDL-C、IMA联合检测可为早期干预提供参考依据,可作为病情评估及预后判断的血清学指标。 Objective To investigate the correlation between small dense low-density lipoprotein cholesterol(sdLDL-C)and ischemic modified protein(IMA)in acute ischemic stroke.Methods 101 cases of acute stroke patients in our hospital from January 2021 to May 2021 were selected as the research object,and 47 cases of healthy physical examination in our hospital during the same period were selected as the control group.The serum levels of sdLDL-C and IMA were compared between the two groups,and their correlation with the severity of the disease(national institutes of health stroke scale(NIHSS)was analyzed).Results The expression levels of sdLDL-C and IMA in the case group were higher than those in the control group,and the differences were statistically significant.The difference of sdLDL-C and IMA in patients with different degree of nerve defect was statistically significant.Smoking,hypertension,diabetes,sdLDL-C,IMA and low density lipoprotein(LDL)are risk factors for ischemic stroke,and the risk levels of LDL,sdLDL-C and IMA are high.According to ROC curve,it was found that IMA and sdLDL-C combined detection had better efficacy.At the same time,smoking,diabetes,LDL,IMA and sdLDL-C were positively correlated with NIHSS score,and sdLDL-C and IMA were the most closely correlated.Conclusion Small Dense Low-Density Lipoprotein Cholesterol is an important risk factor for ischemic stroke.The combined detection of sdLDL-C and IMA can provide a reference for early intervention,and can be used as a serological index for disease assessment and prognosis.
