DHM评价糖尿病患者左房容积和功能的变化

Evaluating the Changes of Volume and Function of the LeftAtrium in Diabetic Patients with Dynamic Heart Model

目的 探讨动态心脏模型成像(dynamic heart model, DHM)技术评价糖尿病患者左心房容积及功能的变化。方法 随机选取2022年4月至2022年11月锦州医科大学附属第一医院诊疗糖尿病患者100例,分为单纯糖尿病组(A组)60例、糖尿病心肌病组(B组)40例,同期选取正常对照组(C组)90例,均采用DHM技术检测左房最大容积(maximum volume of left atrium, LAVmax)、左房最小容积(minimum volume of the left atrium, LAVmin),左房射血分数(left atrial ejection fraction, LAEF)及左房容积指数(left atrium volume index, LAVI)应用二维超声检测常规心脏结构和功能,比较各组间左房容积和功能参数的变化。结果 B组左房前后径(left atrial diameter, LAD)较A组和C组增大、A组和B组E/e′较C组增大,差异均有统计学意义(P<0.05);B组LAVmax、LAVmin、LAVI及LAEF较C组均明显增加,较A组均增加;A组LAVmax、LAVI及LAEF较C组增加,差异均有统计学意义(P<0.05)。结论 糖尿病患者左房容积增大、机械功能受损,合并心肌病变者受损更加显著;DHM可早期准确评估糖尿病病患者左房容积和功能,对指导临床诊治有重要价值。

Objective To explore the changes of volume and function of the left atrium in diabetic patients with Dynamic Heart Model(DHM).Methods A total of 100 patients with diabetes diagnosed and treated in the first affiliated hospital of Jinzhou Medical University from April to November 2022 were randomly selected and divided into simple diabetes group(group A)with 60 cases,diabetic cardiomyopathy group(group B)with 40 cases,and normal control group(group C)with 90 cases.DHM technology was used to detect maximum volume of left atrium(LAVmax)and minimum volume of the left atrium(LAVmin),left atrial ejection fraction(LAEF)and left atrium volume index(LAVI).Conventional cardiac structure and function were measured by two-dimensional ultrasound,and left atrial volume and function parameters were compared among all groups.Results The left atrial diameter(LAD)in group B increased as compared with group A and C,and the E/e′in groups A and B increased as compared with group C,with statistical significance(P<0.05);LAVmax,LAVmin,LAVI and LAEF in group B were significantly increased compared with group C,and significantly increased compared with group A;LAVmax,LAVI and LAEF in group A were higher than those in group C,and the differences were statistically significant(P<0.05).Conclusion Left atrial volume increased and mechanical function impaired in patients with diabetes,especially in patients with cardiomyopathy.DHM can accurately evaluate the left atrial volume and function of diabetic patients in early stage,which is of great value in guiding clinical diagnosis and treatment.
