持续葡萄糖监测在糖尿病患者低血糖监测中的应用

摘要 目的通过持续葡萄糖监测的临床应用,探求其在诊断低血糖中的价值.方法回顾性分析了2021年1月-2022年12月慈溪市人民医院收治的34例糖尿病患者使用持续葡萄糖监测情况.结果研究报告提示,通过持续葡萄糖监测可发现低血糖现象并不少,但是往往不被认知,从中可以看到一些特殊的糖尿病现象,如:黎明现象、黄昏现象.1型糖尿病发生低血糖的风险高于2型糖尿病.胰岛素促泌剂及胰岛素是诱发低血糖的主要药物因素,因此,糖尿病患者使用此类药物时应严格遵医嘱用药,其最佳方案为从小剂量开始,而后根据患者血糖谱逐步增加剂量.夜间睡眠时是低血糖的高发时段.结论持续葡萄糖监测在一定程度上弥补了自我血糖监测(SMBG)和传统血糖监测HbA1c的缺陷,实现了全面、连续监测血糖水平波动的目的,有效地避免了传统监测方法存在的盲区,可更好地帮助临床医生及时诊断低血糖,制定出精准化、个体化的降糖方案,是未来糖尿病葡萄糖监测技术的发展方向. Objective To explore the value of continuous glucose monitoring in the detection of hypoglycemia through its clinical application.Methods The continuous glucose monitoring of 34 diabetes patients in our hospital from January 2021 to December 2022 was analyzed retrospectively.Results The research report suggests that through continuous glucose monitoring,we find a lot of hypoglycemia,but it is often not recognized,which can let us see some special diabetes phenomena,such as dawn phenomenon and dusk phenomenon.The risk of hypoglycemia in Type 1 diabetes was higher than that in type 2 diabetes.Insulin secretagogues and insulin are the main drug factors that predispose to hypoglycemia,so patients with diabetes should use these drugs strictly as directed by their doctor,and the best regimen is to start with a small dose and gradually increase the dose according to the patient's blood glucose profile.Nighttime sleep is a high incidence period of hypoglycemia.Conclusion To a certain extent,continuous glucose monitoring makes up for the shortcomings of self-glucose monitoring(SMBG)and traditional blood glucose detection HbA1c,realizes the purpose of comprehensive and continuous monitoring of blood glucose level fluctuations,effectively avoids the blind spot existing in traditional monitoring methods,better helps clinicians diagnose hypoglycemia in time,and formulates accurate and individualized hypoglycemic programs,which is the development direction of diabetes glucose monitoring technology in the future.
