利湿消肿汤在下肢闭合性骨折术后肿胀治疗中的作用分析

Effect of Detumescence Decoction on Swelling of Lower Extremity after Closed Fracture

目的:分析下肢闭合性骨折患者合并术后肿胀采用中医药利湿消肿汤的治疗价值。方法:抽取我院2021年1月-2022年8月诊断结果均为下肢闭合性骨折术后合并下肢肿胀病例74例,借助随机数字表法分组,即对照组、观察组,均为37例,对照组为常规治疗,同期观察组联合应用中医药利湿消肿汤进行治疗,对于2组患者的临床疗效、施治前后的患者肿胀、疼痛以及症状消失时间和术后并发症率等进行比较。结果:施治前2组下肢VAS疼痛程度评分相比差异甚微,P0.05,与对照组相比,施治后1天、3天、7天的下肢VAS疼痛程度评分均为观察组较低,P<0.05;施治前2组患肢肿胀部位最大周径相比差异甚微,P>0.05,与对照组相比,施治后1天、3天、7天的患肢肿胀部位最大周径均为观察组较小,P<0.05;治疗总有效率比较中,观察组为97.30%、对照组为83.78%,P<0.05;与对照组相比,术后患肢肿胀、疼痛以及皮下瘀斑的消失时间均为观察组较短,P<0.05;术后并发症率比较中,观察组为0.00%、对照组为8.11%,P<0.05;治疗前2组的患肢轻度肿胀、中度肿胀以及重度肿胀率相比均差异较小,P>0.05,治疗后与对照组相比患肢轻度肿胀率为观察组更高,P<0.05,而中度肿胀率和重度肿胀率均为观察组更低,P<0.05。结论:对于下肢闭合性骨折患者的术后肿胀情况采用中医药利湿消肿汤治疗可显著提升疗效、加快肿胀消退,并减少并发症。

Objective:To analyze the therapeutic value of traditional Chinese medicine detumescence decoction for patients with lower extremity closed fracture complicated with postoperative swelling.Methods:The cases were selected from January 2021 to August 2022 in our hospital.The diagnosis results were all closed fracture of lower limbs complicated with swelling of lower limbs,74 cases were di-vided by random number table method,i.e.,37 cases were in the control group and 37 cases in the observation group.The control group was treated with conventional treatment,and the observation group was treated with traditional Chinese medicine deswelling decoction dur-ing the same period.The clinical efficacy,swelling and pain before and after treatment and the rate of postoperative complications were compared between the two groups.Results:Before treatment,the lower extremity VAS pain score of the two groups had little difference(P>0.05).Compared with the control group,the lower extremity VAS pain score of the observation group was lower(P<0.05)at 1,3 and 7 days after treatment.Before treatment,there was little difference in the maximum circumference of the swelling site of the af-fected limb between the two groups(P>0.05).Compared with the control group,the maximum circumference of the swelling site of the affected limb on day 1,3 and 7 after treatment was smaller in the observation group(P<0.05).The total effective rate was 97.30%in the observation group and 83.78%in the control group,P<0.05.Compared with the control group,the disappearance time of postoperative swelling,pain and subcutaneous ecchymosis was shorter in the observation group(P<0.05).The postoperative complica-tion rate was 0.00%in the observation group and 8.11%in the control group(P<0.05);Before treatment,the incidence of mild swelling,moderate swelling and severe swelling of the affected limb in the 2 groups had small differences(P>0.05).After treatment,compared with the control group,the rate of mild swelling of the affected limb was higher in the observation group(P<O.05),while the rate of moderate swelling and severe swelling were lower in the observation group(P<0.05).Conclusion:For the postoperative swlling of patients with closed fracture of lower extremity,the treatment of traditional Chinese medicine detumescence decoction can sig-nificantly improve the curative effect,accelerate the regression of swelling,and reduce complications.
