中药封包联合中频脉冲电对椎间盘源性腰痛患者疼痛及功能康复的影响

Effect of traditional Chinese medicine package combined with medium frequency pulse electricity on pain and functional rehabilitation of patients with discogenic low back pain

目的探讨中药封包联合中频脉冲电对椎间盘源性腰痛患者疼痛及功能康复的影响。方法选取90例椎间盘源性腰痛患者作为研究对象,应用随机数字表法,依照1∶1比例将患者分为观察组与对照组,每组45例。对照组患者应用非甾体抗炎药物联合中频脉冲电治疗,观察组患者在对照组基础上增加中药封包治疗。对比两组患者治疗前后的疼痛程度,炎性因子及P物质水平变化情况,功能康复水平。结果治疗后,两组患者视觉模拟评分法(VAS)、简式疼痛评分量表(MPQ)评分均明显低于本组治疗前,且观察组VAS、MPQ评分分别为(3.85±1.54)、(3.50±1.14)分,低于对照组的(4.75±1.23)、(5.60±1.86)分,差异有统计学意义(P<0.05)。治疗后,两组患者肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-3(MMP-3)、白细胞介素-1β(IL-1β)、P物质水平均低于本组治疗前,且观察组TNF-α、MMP-3、IL-1β、P物质水平分别为(2.36±0.44)ng/L、(23.54±3.49)ng/ml、(0.26±0.11)ng/L、(64.59±11.24)pg/ml,均明显低于对照组的(2.67±0.68)ng/L、(25.32±4.11)ng/ml、(0.32±0.08)ng/L、(81.12±12.64)pg/ml,差异有统计学意义(P<0.05)。治疗后,两组患者直腿抬高角度大于本组治疗前,Oswestry功能障碍指数(ODI)评分低于本组治疗前,观察组直腿抬高角度(62.20±6.05)°大于对照组的(58.30±7.28)°,ODI评分(8.90±3.69)分低于对照组的(11.60±5.72)分,差异有统计学意义(P<0.05)。结论对椎间盘源性腰痛患者应用中药封包联合中频脉冲电治疗能够减轻患者腰部疼痛,降低患者机体炎症反应,提升功能康复水平,值得临床应用推广。

Objective To discuss the effect of traditional Chinese medicine package combined with medium frequency pulse electricity on pain and functional rehabilitation of patients with discogenic low back pain.Methods A total of 90 patients with discogenic low back pain were studied and randomly divided into the observation group and the control group in a 1∶1 ratio according to random numerical table,with 45 cases in each group.The control group was treated with non-steroidal anti-inflammatory drugs and medium frequency pulse electricity,and the observation group was treated with Chinese medicine encapsulation on the basis of the control group.Both groups were compared in terms of pain level,inflammatory factors and substance P levels before and after treatment,and the functional rehabilitation level.Results After treatment,the scores of Visual Analogue Scale(VAS)and Mcgil Pain Questionnaire(MPQ)in both groups were significantly lower than those before treatment in this group;the scores of VAS and MPQ in the observation group were(3.85±1.54)and(3.50±1.14)points,which were lower than those of(4.75±1.23)and(5.60±1.86)points in the control group;the differences were statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),matrix metalloproteinase-3(MMP-3),interleukin-1β(IL-1β)and substance P in both groups were lower than those before treatment in this group;the levels of TNF-α,MMP-3,IL-1βand P in the observation group were(2.36±0.44)ng/L,(23.54±3.49)ng/ml,(0.26±0.11)ng/L and(64.59±11.24)pg/ml,which were obviously lower than those of(2.67±0.68)ng/L,(25.32±4.11)ng/ml,(0.32±0.08)ng/L and(81.12±12.64)pg/ml in the control group;the differences were statistically significant(P<0.05).After treatment,the straight leg raise angle in both groups was greater than that before treatment in this group,and the Oswestry Disability Index(ODI)score was lower than that before treatment in this group;the straight leg raise angle of(62.20±6.05)°in the observation group was greater than that of(58.30±7.28)°in the control group;ODI score of(8.90±3.69)points in the observation group was lower than that of(11.60±5.72)points in the control group;the differences were statistically significant(P<0.05).Conclusion The application of traditional Chinese medicine package combined with medium frequency pulse electricity in patients with discogenic low back pain can reduce the low back pain,reduce the inflammatory reaction and improve the level of functional rehabilitation,which is worthy of clinical application and promotion.
