生化检验质量控制管理在糖尿病患者生化指标测定中的应用分析

摘要 目的分析生化检验质量控制管理在糖尿病患者生化指标测定中的应用价值。方法110例2型糖尿病患者作为研究对象,按照就诊顺序分为控制组和参照组,每组55例。两组均接受两个阶段的生化指标测定,第一阶段,所有受试者均于生化检验质量控制管理下开展相关指标检测;第二阶段,参照组不接受生化检验质量控制管理,控制组接受生化检验质量控制管理。对比两组生化指标[糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(2 h PG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)],生化指标变异系数以及偏倚。结果第一阶段,两组HbA1c、FBG、2 h PG、TC、TG、LDL-C、HDL-C水平对比差异无统计学意义(P>0.05);控制组第一阶段和第二阶段的HbA1c、FBG、2 h PG、TC、TG、LDL-C、HDL-C水平对比差异无统计学意义(P>0.05)。参照组第二阶段的HbA1c、FBG、2 h PG、TC、TG、LDL-C、HDL-C水平均高于本组第一阶段,差异具有统计学意义(P<0.05)。第二阶段,控制组的HbA1c、FBG、2 h PG、TC、TG、LDL-C、HDL-C分别为(8.41±1.54)%、(7.73±1.63)mmol/L、(10.51±2.16)mmol/L、(6.13±0.92)mmol/L、(2.38±0.76)mmol/L、(4.21±0.59)mmol/L、(1.46±0.48)mmol/L,均低于参照组的(9.04±1.33)%、(8.66±1.49)mmol/L、(11.64±1.61)mmol/L、(7.15±0.65)mmol/L、(3.23±0.56)mmol/L、(5.18±0.50)mmol/L、(2.24±0.36)mmol/L,差异具有统计学意义(P<0.05)。控制组第一阶段的生化达标率为98.18%(54/55),与第二阶段的94.55%(52/55)对比差异无统计学意义(P>0.05)。参照组第一阶段的生化达标率为96.36%(53/55),高于第二阶段的85.45%(47/55),差异有统计学意义(P<0.05)。结论针对糖尿病患者开展生化检验质量控制管理,能有效提升患者生化指标测定达标率,这一方案值得进一步在临床之内推广以及应用。 Objective To analyze the application value of biochemical test quality control management in the determination of biochemical indexes in diabetic patients.Methods A total of 110 patients with type 2 diabetes mellitus were studied and divided into control group and reference group according to the order of treatment,with 55 cases in each group.Both groups underwent two stages of biochemical test;in the first stage,all subjects were tested for biochemical quality control related indicators;in the second stage,the reference group did not accept biochemical test quality control management,while the control group accepted biochemical test quality control management.Both groups were compared in terms of biochemical indices[glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),2 h postprandial plasma glucose(2 h PG),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)],variation coefficient and bias of biochemical indexes.Results At the first stage,there was no statistically significant difference in the levels of HbA1c,FBG,2 h PG,TC,TG,LDL-C and HDL-C between the two groups(P>0.05).In the control group,there was no statistically significant difference in the levels of HbA1c,FBG,2 h PG,TC,TG,LDL-C and HDL-C between the first stage and the second stage(P>0.05).The levels of HbA1c,FBG,2 h PG,TC,TG,LDL-C and HDL-C at the second stage of the reference group were higher than those at the first stage,and the differences were statistically significant(P<0.05).At the second stage,HbA1c,FBG,2 h PG,TC,TG,LDL-C and HDL-C in the control group were(8.41±1.54)%,(7.73±1.63)mmol/L,(10.51±2.16)mmol/L,(6.13±0.92)mmol/L,(2.38±0.76)mmol/L,(4.21±0.59)mmol/L and(1.46±0.48)mmol/L,which were lower than those of(9.04±1.33)%,(8.66±1.49)mmol/L,(11.64±1.61)mmol/L,(7.15±0.65)mmol/L,(3.23±0.56)mmol/L,(5.18±0.50)mmol/L and(2.24±0.36)mmol/L in the reference group,and the differences were statistically significant(P<0.05).The biochemical compliance rate of the control group at the first stage was 98.18%(54/55),which had no statistical significance compared with that of 94.55%(52/55)at the second stage(P>0.05).The biochemical compliance rate of the reference group at the first stage was 96.36%(53/55),which was higher than that of 85.45%(47/55)at the second stage,and the difference was statistically significant(P<0.05).Conclusion The biochemical test quality control management for diabetic patients can effectively improve the up-to-standard rate of biochemical indexes,which is worthy of further promotion and application in clinical practice.
