针灸治疗对疼痛动物模型中TRPV亚家族蛋白表达量影响的Meta分析

Effects of Acupuncture and Massage Therapy on TRPV Subfamily Protein Expression in Different Pain Animal Models:A Meta Analysis

摘要 目的:本次研究通过Meta分析法分析针灸治疗对不同疼痛动物模型中TRPV表达量的影响。方法:文献检索PubMed、Embase、Web of Science、CNKI、Wanfang数据库,查找针灸疗法影响TRPV表达量的相关动物实验。对纳入的文献进行系统评价,进行Meta分析,旨在拓展针推疗法治疗各类疾病机制的证据,为后期研究提供参考。结果:共纳入文献的18篇,包括4篇英文文献,14篇中文文献,都采取了随机对照实验,未提及盲法的使用,18篇文献分别测量了TRPV家族不同蛋白的表达。将18篇文献数据提取出来,通过Meta分析发现,针灸疗法对于TRPV家族的表达下降,有统计学意义,但是18篇文献的异质性较高(SMD=-2.28,95%CI=-2.95~-1.61,P<0.01;异质性χ^(2)=404.04,I 2=90%,随机效应模型)。根据不同的动物模型分组,无论是炎性疼痛还是神经病理性疼痛,TRPV的表达下降都具有统计学意义,而癌性疼痛亚组中,针灸对于TRPV表达无统计学意义,并且3个亚组间的异质性较小;根据不同TRPV蛋白分组,分为4组,针灸对于TRPV1、TRPV4的表达下降有统计学意义,而对于TRPV2、TRPV3的表达无统计学意义。只有1篇文献检测了TRPV3的表达。结论:针灸镇痛临床疗效确切,针灸镇痛的机制与调节TRPV1和TPRV4的活性有关。 Objective This study analyzed the influence of acupuncture therapy on the expression of TRPV in different pain animal models by meta analysis.Methods We searched for relevant animal experiments on the influence of acupuncture therapy about TRPV expression though PubMed,Embase,CNKI and Wanfang databases.We evaluted the the included literatures and performed the on the extracted data by meta-analysis,aiming to expand the evidence of acupuncture therapy in treating various pain diseases and provide reference for later research.Results A total of 18 articles were included with 4 English articles and 14 Chinese articles.The included aticles were all randomized controlled experiments,but no mentioning the use of blind method,and the expression of different proteins in the TRPV family was measured in 18 articles.We extracted the data from 18 literatures and The meta-analysis result showed that acupuncture therapy decreased the expression of TRPV family,which was statistically significant,but the heterogeneity of 18 literatures was high(SMD=-2.28,95%CI=-2.95~-1.61,P<0.01;Heterogeneity χ^(2)=404.04,I 2=90%,random effect model).According to different animal model subgroups,the decreased expression of TRPV was statistically significant in both inflammatory pain and neuropathic pain,while acupuncture had no statistically significant effect on TRPV expression in cancer pain subgroup.There was little heterogeneity among the three subgroups.According to different TRPV protein groups,acupuncture was divided into 4 groups.The decreased expression of TRPV1 and TRPV4 had statistical significance,while the expression of TRPV2 and TRPV3 had no statistical significance.TRPV3 expression was detected in only one study.Conclusion Acupuncture therapy can be used as supplementary treatment in various pain diseases.The analgesic mechanism of acupuncture therapy is related to the regulation of TRPV1 and TPRV4.
