复方苦参注射液预防食管癌急性放射性皮炎临床观察

Clinical Observation of Compound Kushen Injection in Preventing Acute Radiation Dermatitis of Esophageal Carcinoma

目的观察复方苦参注射液防治食管癌急性放射性皮炎的临床观察。方法根据治疗方案的不同将医院纳入的80例中晚期食管癌患者进行分组,分为观察组40例和对照组40例,对照组给予患者常规放疗,观察组则在对照组基础上给予复方苦参注射液进行治疗。比较两组患者急性放射性皮炎的分级及发生率、急性放射性皮炎反应评分、皮肤疼痛视觉模拟评分(visual analogue score,VAS)和生活质量评分。结果治疗后,观察组发生Ⅰ级放射性皮炎的病例数为31(77.5%),高于对照组发生Ⅰ级放射性皮炎的病例数22(55.0%),差异具有统计学意义(P<0.05);观察组发生Ⅱ级、Ⅲ级的放射性皮炎病例数8(20.0%)、1(2.5%),高于对照组发生Ⅱ级、Ⅲ级的放射性皮炎的病例数16(40.0%)、2(5.0%),但差异无统计学意义(P>0.05)。治疗后,观察组RISRAS评分低于对照组,说明观察组患者皮肤损伤程度低,其中治疗第1周和第2周两组差异无统计学意义(P>0.05),治疗第3周、第4周和第5周两组差异有统计学意义(P<0.05)。治疗后,观察组VAS评分较对照组降低(P<0.05),说明疼痛有所缓解。治疗后,观察组KPS评分有所改善及稳定的患者较对照组有所升高(P<0.05),观察组KPS评分有所下降的患者较对照组有所下降(P<0.05)。结论食管癌患者在放疗期间联合复方苦参注射液治疗可降低患者急性放射性皮炎的发生率,减轻患者严重程度及疼痛程度,且提高患者生活质量。

Objective To observe the clinical effect of Compound Kushen Injection on acute radiation dermatitis of esophageal carcinoma.Methods Eighty patients with advanced esophageal carcinoma were divided into the observation group(n=40)and the control group(n=40).Patients in control group were treated with conventional radiotherapy,while patients in the observation group were treated with Compound Kushen Injection on the basis of the control group.The score of acute radiation dermatitis,visual analogue score(VAS)and quality of life were compared between the two groups.Results After treatment,the incidence of grade Ⅰ radiation dermatitis in the observation group was 31(77.5%),which was higher than that in the control group(22(55.0%)),the difference was statistically significant(P<0.05);8(20.0%)and 1(2.5%)cases of grade Ⅱ and Ⅲ radiation dermatitis occurred in the observation group,which were higher than 16(40.0%)and 2(5.0%)cases of grade Ⅱ and Ⅲ radiation dermatitis in the control group,but the difference was not significant(P>0.05).After treatment,the RISRAS score in the observation group was lower than that in the control group,indicating that the degree of skin injury in the observation group was lower,and there was no significant difference between the two groups at the first and second week of treatment(P>0.05),there were significant differences between the two groups at the 3rd,4th and 5th week(P<0.05).After treatment,the VAS score in the observation group was lower than that in the control group(P<0.05),indicating that the pain was relieved.After treatment,the patients whose KPS scores were improved and stable in the observation group were higher than those in the control group(P<0.05),and the patients whose KPS scores were lower in the observation group were lower than those in the control group(P<0.05).Conclusion The combined therapy of Compound Kushen Injection and radiotherapy can reduce the incidence of acute radiation dermatitis,relieve the severity and pain of the patients,and improve the quality of life of the patients.Combined therapy with Compound Kushen Injection during radiotherapy can reduce the incidence of acute radiation dermatitis,relieve the severity and pain of patients,and improve the quality of life of patients with esophageal carcinoma.
