基于超声监视下胎儿外倒转术矫治臀位临床价值研究

Study on the clinical value of external fetal inversion for breech correction under ultrasonic monitoring

目的探讨基于超声监视下胎儿外倒转术矫治臀位的临床应用价值。方法选择60例臀位妊娠孕妇为研究对象,根据随机数字表法分为对照组和研究组,每组30例。对照组给予传统手动转换,研究组给予超声监视下的胎儿外倒转术。比较两组产妇产后、新生儿并发症发生情况、住院时间及成功转换为头位情况。结果两组产妇产后出血及新生儿窒息发生率比较,差异均无统计学意义(P>0.05);两组住院时间比较,差异无统计学意义(P>0.05);研究组成功转换率为93.3%,对照组成功转换率为83.3%,研究组成功转换率高于对照组,但两组比较差异无统计学意义(P>0.05);研究组转换时间短于对照组(P<0.05)。结论基于超声监视下的胎儿外倒转术矫治臀位的临床应用效果较好,相比传统手动转换具有更高的成功率和更短的操作时间;而且,该技术在母亲和胎儿的并发症发生率方面与传统方法相当。然而,由于样本量较小,仍需进一步研究。

Objective To investigate the clinical application value of external fetal conversion correction for breech based on ultrasonic monitoring.Methods A total of 60 pregnant women with breech pregnancy were selected as the study objects and divided into control group and study group according to random number table method,with 30 cases in each group.The control group was given traditional manual conversion,and the study group was given external fetal conversion under ultrasonic monitoring.The occurrence of postpartum and neonatal complications,length of hospital stay and successful conversion to head position were compared between the two groups.Results There were no significant differences in the incidence of postpartum hemorrhage and neonatal asphyxia between the two groups(P>0.05).There was no significant difference in hospital stay between the two groups(P>0.05).The successful conversion rate of the study group was 93.3%and that of the control group was 83.3%.The successful conversion rate of the study group was higher than that of the control group,but there was no statistical significance between the two groups(P>0.05).The conversion time of study group was shorter than that of control group(P<o.o5).Conclusion The treatment of breech by external fetal conversion under ultrasonic monitoring has better clinical effect,higher success rate and shorter operation time than traditional manual conversion.Moreover,the technique was comparable to traditional methods in terms of complication rates for both mother and fetus.However,due to the small sample size,further research is needed.
