经脐单孔及多孔腹腔镜妇科手术的临床效果观察

Observation of clinical effect of transumbilical single-hole and multi-hole laparoscopic gynecological surgery

目的探讨经脐单孔与多孔腹腔镜手术在良性妇科疾病(输卵管妊娠、卵巢囊肿、子宫肌瘤、卵巢畸胎瘤)患者中的应用效果。方法选择66例良性妇科疾病患者为研究对象,根据随机数字表法分为研究组和对照组,每组33例。研究组给予经脐单孔腹腔镜手术治疗,对照组给予传统多孔腹腔镜手术治疗,比较两组围手术期相关指标、术后疼痛情况及满意度。结果两组术中出血量比较,差异无统计学意义(P>0.05);研究组手术时长长于对照组,术后首次下床活动、肛门排气及住院时间均短于对照组(P<0.05);术后24h、48h及72h,研究组Prince-Henry疼痛评分均低于对照组(P<0.05);研究组术后切口总满意度高于对照组(P<0.05)。结论相比传统多孔腹腔镜手术治疗,经脐单孔腹腔镜手术治疗良性妇科疾病可有效缓解术后疼痛,促进患者术后康复,提高切口满意度,值得临床推广与应用。

Objective To investigate the application effect of transumbilical single-pore and multi-pore laparoscopic surgery in benign gynecological diseases(tubal pregnancy,ovarian cyst,uterine fibroid,ovarian teratoma).Methods A total of 66 patients with benign gynecological diseases were selected as the study objects and divided into study group and control group according to random number table method,33 cases in each group.The study group received transumbilical single-hole laparoscopic surgery,and the control group received traditional porous laparoscopic surgery.The perioperative indexes,postoperative pain and satisfaction of the two groups were compared.Results There was no significant difference in intraoperative blood loss between the two groups(P>0.05).The operation time of the study group was longer than that of the control group,and the first time of getting out of bed,anal exhaust and hospitalization time were shorter in the study group than in the control group(P<0.05).The Prince-Henry pain scores in the study group were lower than those in the control group at 24 h,48 h and 72 h after surgery(P<0.05).The total incision satisfaction of the study group was higher than that of the control group(P<0.05).Conclusion Compared with traditional porous laparoscopic surgery,transumbilical single-hole laparoscopic surgery for benign gynecological diseases can effectively relieve postoperative pain,promote postoperative rehabilitation of patients,and improve incision satisfaction,which is worthy of clinical promotion and application.
