炔雌醇环丙孕酮联合二甲双胍治疗对多囊卵巢综合征排卵率的影响

Effect of ethinylestradiol cyproterone combined with metformin on ovulation rate in polycystic ovary syndrome

目的探究炔雌醇环丙孕酮联合二甲双胍治疗对多囊卵巢综合征(PCOS)排卵率的影响,为患者治疗提供参考。方法选取294例PCOS患者,按照随机数字表法将患者分为对照组与研究组,各147例。对照组给予炔雌醇环丙孕酮治疗,研究组在对照组基础上联合二甲双胍治疗。对比两组血清性激素水平、卵泡发育情况、排卵率及妊娠率。结果治疗前,两组促黄体生成素(LH)、雌二醇(E_(2))、卵泡刺激素(FSH)水平比较,差异无统计学意义(P>0.05);治疗后,研究组患者LH水平低于对照组,E_(2)、FSH水平高于对照组,差异有统计学意义(P<0.05)。两组患者成熟卵泡直径、子宫内膜厚度比较,差异无统计学意义(P>0.05),研究组成熟卵泡数量显著多于对照组,差异有统计学意义(P<0.05)。研究组患者排卵率85.71%、妊娠率39.46%均显著高于对照组的71.43%、21.77%,差异有统计学意义(P<0.05)。结论炔雌醇环丙孕酮联合二甲双胍在PCOS治疗中应用效果理想,可以有效改善患者性激素水平,提升成熟卵泡数量,提高患者排卵率、妊娠率,值得临床推广。

Objective To explore the effect of ethoestradiol cyproterone combined with metformin on ovulation rate of polycystic ovary syndrome(PCOS),and to provide reference for the treatment of patients.Methods 294 patients with PCOS were selected and divided into control group and study group with 147 cases in each group according to random number table method.The control group was treated with ethinylestradiol cyproterone,and the study group was treated with metformin based on the control group.Serum sex hormone levels,follicle development,ovulation rate and pregnancy rate were compared between the two groups.Results Before treatment,there was no significant difference in the levels of luteinizing hormone(LH),estradiol(E_(2))and follicle-stimulating hormone(FSH)between the two groups(P>0.05).After treatment,LH levels in the study group were lower than those in the control group,and E_(2) and FSH levels were higher than those in the control group,with statistical significance(P<0.05).There was no significant difference in mature follicle diameter and endometrial thickness between the two groups(P>0.05),while the number of mature follicles in the study group was significantly higher than that in the control group(P<0.05).Ovulation rate of 85.71%and pregnancy rate of 39.46%in the study group were significantly higher than 71.43%and 21.77%in the control group,with statistical significance(P<0.05).Conclusion Ethoestradiol cyproterone combined with metformin has an ideal effect in the treatment of Pcos,which can effectively improve the level of sex hormones in patients,increase the number of mature follicles,and increase the ovulation rate and pregnancy rate of patients,which is worthy of clinical promotion.
